摘 要:目的:分析机械按压和人工按压对院外心脏骤停患者在院前急救中的复苏效果,评价机械按压在院前急救中的应用价值。方法:本研究为病例对照研究,研究对象为宜兴市急救中心于2021年6月~2022年6月救治的122例院外心脏骤停患者,根据不同的按压方式分机械按压组和人工按压组。机械按压组为观察组,人工按压组为对照组。比较两组院前自主循环恢复率及不良事件的发生率,比较两组患者心肺复苏后的收缩压、血氧饱和度及呼末二氧化碳分压。结果:共有132例院外心脏骤停符合入组标准,其中观察组65例,对照组67例。两组患者的收缩压[63(60,86)mmHg,57(53,64)mmHg,P<0.05],血氧饱和度[90%(87%,93%),79%(76%,93%),P<0.05],呼末二氧化碳[23(22,32)mmHg,22(19,23)mmHg,P<0.05],自主循环恢复率(29.23%,14.93%,P<0.05),不良事件发生率(10.76%,16.41%,P>0.05)。结论:相比人工胸外按压,机械按压可稳定院外心脏骤停患者的生命体征,提升院前自主循环恢复率。Objective:To analyze the resuscitation effects of mechanical and manual compressions on patients with out-of-hospital cardiac arrest(OHCA)in prehospital emergency,and to evaluate the application value of mechanical compressions in prehospital emergency.Methods:This study was a case-control study,and the subjects were 122 OHCA patients treated in Yixing Emergency Center from June 2021 to June 2022.The patients were divided into mechanical compression group and manual compression group according to different compression methods.The mechanical group was the research group and the manual pressing group was the control group.The rate of restoration of spontaneous circulation(ROSC)and the incidence of adverse time were compared between the two groups.The systolic blood pressure,oxygen saturation and end-breath partial carbon dioxide pressure after cardiopulmonary resuscitation were compared between the two groups.Results:A total of 132 patients with out of hospital cardiac arrest met the inclusion criteria,including 65 in the study group and 67 in the control group.Systolic blood pressure[63(60,86)mmHg,57(53,64)mmHg,P<0.05],oxygen saturation[90%(87%,93%),79%(76%,93%),P<0.05],end expiratory carbon dioxide[23(22,32)mmHg,22(19,23)mmHg,P<0.05],ROSC recovery rate(29.23%,14.93%,P<0.05),adverse event rate(10.76%,16.41%,P>0.05).Conclusion:Compared with artificial chest compressions,mechanical compressions can stabilize the vital signs of patients with OHCA and improve the rate of pre-hospital ROSC.Mechanical compressions are worth popularizing in pre-hospital emergency institutions.
