微血管减压手术治疗面肌痉挛的临床效果及相关影响因素分析

摘　　要：目的分析微血管减压手术(MVD)治疗面肌痉挛(HFS)的临床效果及其影响因素。方法回顾性分析2015年9月至2022年6月在我院接受MVD治疗的57例HFS患者的临床资料,统计临床疗效,采用单因素分析与Logistic多因素回归分析找出影响疗效的相关因素。结果57例患者中,治疗有效50例,治疗无效7例。治疗无效组的糖尿病病史、血管攀综合征、穿通支、血管压迹、单点垫离的患者占比均显著高于治疗有效组(P<0.05)。Logistic多因素回归分析显示,血管攀综合征、穿通支、血管压迹、垫离类型是MVD治疗HFS患者临床效果的独立影响因素(OR>1,P<0.05)。结论仍有部分HFS患者采用MVD治疗效果不佳,血管攀综合征、穿通支、血管压迹、垫离类型等因素均可对临床疗效产生影响。Objective To explore the clinical effect of microvascular decompression(MVD)in the treatment of hemifacial spasm(HFS)and its related influencing factors.Methods The clinical data of 57 patients with HFS who received MVD treatment in our hospital from September 2015 to June 2022 were retrospectively analyzed.The clinical efficacy was counted,and univariate analysis and Logistic multivariate regression analysis were used to find out the related factors affecting the efficacy.Results Of the 57 patients,50 were effective and 7 were ineffective.The proportions of patients with diabetes history,vascular climbing syndrome,perforating branch,vascular impression and single point separation in the ineffective treatment group were significantly higher than those in the effective treatment group(P<0.05).Logistic multivariate regression analysis showed that vascular climbing syndrome,perforating branch,vascular impression and separation type were independent influencing factors of clinical effect of MVD in the treatment of HFS patients(OR>1,P<0.05).Conclusions There are still some HFS patients have poor MVD treatment effect.Vascular climbing syndrome,perforating branch,vascular impression and separation type can affect the clinical efficacy.
