柴桂温胆汤加味治疗脑梗死恢复期阳虚痰瘀证临床研究

摘　　要：目的分析柴桂温胆汤对脑梗死恢复期阳虚痰瘀证患者的临床疗效。方法选择2021年1月-2021年9月的66例患者,随机均分为两组(各33例)。对照组给予西医常规治疗和康复训练,试验组加用柴桂温胆汤治疗。比较两组治疗前后中医症状积分、美国国立卫生研究所卒中量表(NIHSS评分)、Fugl-Meyer运动功能量表(FMA)评分、日常生活活动能力(Barthel指数)及临床疗效。结果治疗14 d后,对照组总有效率78.79%(26/33);试验组总有效率96.97%(32/33)。试验组半身不遂、口舌歪斜、言语不利、主观感觉评分明显低于治疗前,且低于对照组,差异有统计学意义(P<0.05);两组NIHSS评分均低于治疗前,且试验组低于对照组,差异有统计学意义(P<0.05)。治疗14 d后,两组FMA、BI评分均高于治疗前,且试验组高于对照组,差异有统计学意义(P<0.05)。结论柴桂温胆汤运用于脑梗死恢复期阳虚痰瘀证患者的临床治疗,能有效改善脑梗死恢复期患者神经功能和运动功能的恢复,提高患者生活质量,缓解患者临床症状,疗效较好。Objective To analyze the clinical effect of Chaigui Wendan Decoction(柴桂温胆汤)on patients with Yang deficiency and phlegm-blood stasis syndrome in recovery period of cerebral infarction.Methods A total of 66 patients from January 2021 to September 2021 were randomly divided into two groups with 33 cases in each group.The control group was treated with western medicine and rehabilitation training,while the experimental group was treated with Chaigui Wendan Decoction.The TCM symptom score,National Institute of Health stroke scale(NIHSS)score,Fugl-Meyer(FMA)score,activities of daily living(ADL)score and the clinical efficacy were compared between the two groups before and after treatment.Results after 14 days of treatment,the total effective rate was 78.79%(26/33)in the control group and 96.97%(32/33)in the experimental group.The scores of hemiplegia,crooked tongue,adverse speech and subjective feeling in the experimental group were significantly lower than those before treatment and lower than those in the control group(P<0.05).The NIHSS score of the two groups was lower than that before treatment,and the score of the experimental group was lower than that of the control group(P<0.05).After 14 days of treatment,the scores of FMA and Barthel index(BI)in the two groups were higher than those before treatment,and the scores in the experimental group were higher than those in the control group(P<0.05).ConclusionChaigui Wendan Decoction can effectively improve the recovery of neurological and motor function,improve the quality of life and alleviate the clinical symptoms of patients with Yang deficiency and phlegm-blood stasis syndrome in the recovery period of cerebral infarction.
