摘  要:目的分析中国超重肥胖职业人群100天健走日均步行量与体重、BMI、体脂率变化的关系,探讨适合不同超重肥胖特征职业者减肥步行量,以降低职业人群肥胖风险。方法选取中国疾控中心慢病中心2016—2021年健走激励项目中85380名BMI超重肥胖和125240名体脂率超重肥胖职业者作为研究对象,采用多水平线性模型检验日均步行量与体重、BMI、体脂率变化的关系,并进一步以日均步行量作为检验变量,体重是否下降3%,BMI是否下降1个单位,体脂率是否下降1%作为状态变量,采用ROC曲线分别分析BMI和体脂率超重肥胖人群减肥步行推荐量。结果经过100天的健走,与干预前相比,平均步数每增加2000步,体重平均多下降约0.05 kg,BMI平均多下降约0.02 kg/m^(2),体脂率平均多下降约0.04%;ROC曲线分析发现BMI超重肥胖者健走降低肥胖程度的有效切点为15272步/天,体脂率超重肥胖者健走降低肥胖程度的有效切点为15542步/天。结论对于BMI和体脂率超重肥胖职业者而言,健走步行量均与体重、BMI、腰围变化呈正向相关,15000步/天均可以作为两类人群改善肥胖从而降低慢性病风险的健走参考量。Objective To analyze the relationship between daily walking amount and the change of body weight,BMI and body fat rate of Chinese overweight and obese occupational population during 100-day healthy walking,and to explore the amount of weight-loss walking suitable for different overweight and obese professionals,so as to reduce the risk of obesity in occupational population.Methods A total of 85380 overweight and obese people with BMI and 125240 overweight and obese people with body fat percentage were enrolled in the Healthy Walking Incentive Program of Chronic Disease Center of China CDC in 2016-2021.A multi-level linear model was used to test the relationship between daily walking amount and changes in body weight,BMI and body fat percentage,and the daily walking amount was further used as the test variable,the body weight decreased by 3%,BMI decreased by 1 unit and body fat rate decreased by 1%as status variables.ROC curve was used to analyze the recommended weight-loss walking amount for overweight and obese people with BMI and body fat rate respectively.Results After 100 days of walking,compared with before intervention,body weight,BMI and body fat percentage decreased by 0.05 kg,0.02 kg/m^(2) and 0.04%for every 2000 steps increase.ROC curve analysis showed that the effective cut-off point of walking to reduce the degree of obesity in overweight obese people with BMI was 15272 steps/day,and the effective cut-off point of walking to reduce the degree of obesity in overweight obese people with body fat percentage was 15542 steps/day.Conclusion For overweight and obese people with BMI and body fat rate,the amount of walking is positively correlated with the changes of body weight,BMI and waist circumference.15000 steps per day can be used as the reference amount of walking to improve obesity and reduce the risk of chronic diseases in the two groups.
