摘  要:目的分析45岁及以上中老年人握力与抑郁状态变化的关系。方法基于中国健康与养老调查2015年和2018年的数据,共纳入2760名轻中度抑郁的中老年人,将其握力划分为3个等级,并采用多元logistic回归模型分析不同握力水平对轻中度抑郁状态变化的影响。结果2760名轻中度抑郁的中老年人群中,1095(39.7%)名恢复至无抑郁状态,342(12.4%)名进展为重度抑郁,1323(47.9%)名仍为轻中度抑郁。高握力水平能够促进轻中度抑郁症状的中老年人恢复至无抑郁状态(OR=1.700,95%CI=1.305~2.215),且能够降低其进展为重度抑郁的概率(OR=0.562,95%CI=0.374~0.846)。结论我国中老年人的抑郁状态是动态变化的,高握力水平能够改善中老年人的抑郁症状。Objective To estimate association between grip strength and depression regression and progression among middle-aged and elderly people aged 45 or older.Methods Data were extracted from the China Health and Retirement Longitudinal Study in 2015 and 2018.2760 participants with mild to moderate depression were included,which were divided into three groups according to their grip strength.Multiple logistic regression models were used to analyze the impact of grip strength on the regression and progression of mild to moderate depression.Results Among 2760 middle-aged and elderly people with mild to moderate depression,1095(39.7%)regressed to no depressive symptoms,342(12.4%)progressed to severe depression,and 1323(47.9%)remained in mild to moderation depression.High level of grip strength were not only associated with increased odds of regression to no depressive symptoms(OR=1.700,95%CI=1.305-2.215),but also associated with reduced odds of progression to severe depression(OR=0.562,95%CI=0.374-0.846).Conclusion Depression status of middle-aged and elderly people in China is dynamic.High level of grip strength could improve the depression symptoms of middle-aged and elderly people.
