妊娠期高血压疾病产妇早期母乳喂养体验的质性研究

摘　　要：目的探讨妊娠期高血压疾病产妇早期母乳喂养的体验,为制订相关护理干预策略提供依据。方法采用目的抽样法,选取2021年6月—12月于南京市某三级甲等妇产医院产科分娩后进行母乳喂养的21例妊娠期高血压疾病产妇进行半结构式访谈,采用Colaizzi 7步分析法分析、归纳、提炼资料。结果妊娠期高血压疾病产妇早期母乳喂养体验可归纳为4个主题:①母乳喂养行为建立过程中的冲突;②母乳喂养积极与消极体验并存;③母乳喂养支持系统薄弱;④母乳喂养需求多样化。结论临床护理人员应优化对妊娠期高血压疾病产妇早期母乳喂养决策中的信息支持,重点关注及评估该类产妇母乳喂养的体验及过程中的困难与需求,给予及时的帮助和指导,促进早期母乳喂养行为的建立,并加强出院后母乳喂养的延续性服务,提升其母乳喂养持续时间。Objective To explore the experience of early breastfeeding in postpartum women with hypertensive disorders of pregnancy(HDP),and to provide a basis for nursing and intervention tactics for them.Methods The purposive sampling method was used to select 21 postpartum women with HDP as the research participants.The women with HDP,who were breastfeeding after delivery in the obstetrics department of a tertiary care maternity hospital in Nanjing from June to December 2021 were interviewed by semi-structured interviews,and the data was analyzed by Colaizzi seven-step method.Results 4 themes were extracted.Theme 1:conflict in the establishment of breastfeeding behavior.Theme 2:positive and negative breastfeeding experiences coexist.Theme 3:weak breastfeeding support system.Theme 4:diversification of breastfeeding needs.Conclusion Clinical nurses should optimize information support in early breastfeeding decision making for postpartum women with HDP,focus on and assess the breastfeeding experience and the difficulties and needs in the process of breastfeeding for these women,give them timely help and guidance,promote the establishment of their early breastfeeding behavior,and strengthen the continuity of breastfeeding services after discharge to enhance the duration of their breastfeeding.
