配戴不同角膜塑形镜矫正近视的效果比较

摘　　要：目的比较配戴角膜屈光矫治(CRT)角膜塑形镜和视觉重塑治疗(VST)角膜塑形镜矫正近视的效果。方法回顾性队列研究。选取2017年8月至2019年8月在濮阳市眼科医院就诊的近视青少年204例(204眼),按角膜塑形镜设计不同分为CRT组和VST组,每组各102例(102眼)。随访2年,比较两组的屈光度、眼轴长度、散光、角膜厚度、角膜内皮细胞密度及六角形细胞百分比等参数的变化情况。结果戴镜2年后,CRT组与VST组的屈光度变化量[(-0.82±0.87)D与(-0.83±0.75)D]、眼轴增长[(0.51±0.30)mm与(0.46±0.29)mm]、角膜散光增加量[0.23(0.01,0.42)D与0.22(0.01,0.77)D]、角膜厚度减少量[(0.14±1.22)mm与(0.01±0.13)mm]、角膜内皮细胞密度减少量[(78.85±102.85)个/mm^(2)与(90.22±102.85)个/mm^(2)]、角膜六角形细胞百分率减少量[(0.62±7.08)%与(2.57±11.22)%]、摘镜后角膜染色CCLRU分级[0级、Ⅰ级、Ⅱ级分别为(78眼、16眼、8眼)与(83眼、14眼、5眼)]相比较差异均无统计学意义(P=0.962、0.587、0.290、0.581、0.581、0.362、0.845)。结论佩镜2年时,CRT和VST这两种不同设计的角膜塑形镜对近视的控制作用相似。Objective To compare the efficacy of corneal refractive therapy(CRT)lenses and vision shaping treatment(VST)lenses for myopia control.Methods This was a retrospective cohort study.A total of 204 eyes of 204 adolescents with myopia in the Ophthalmologic Hospital of Puyang from Aug.2017 to Aug.2019 were included.They were divided into 2 groups,based on the different design of orthokeratology lenses,the CRT group and the VST group,with 102 eyes of 102 cases in each group.The changes of diopter,axial length,corneal astigmatism,corneal thickness,corneal endothelial cell density and the percentage of hexagonal cells were compared between the two groups after the 2-year follow-up period.Results After wearing orthokeratology for 2 years,there was no statistical difference in the changes of the corneal refractive power[(-0.82±0.87)D and(-0.83±0.75)D],axial length[(0.51±0.30)mm and(0.46±0.29)mm],corneal astigmatism[0.23(0.01,0.42)D and 0.22(0.01,0.77)D],corneal thickness[(0.14±1.22)mm and(0.01±0.13)mm],cell density of corneal endothelial[(78.85±102.85)cells/mm^(2) and(90.22±102.85)mm^(2)]and percentage of hexagonal cells[(0.62±7.08)%and(2.57±11.22)%]between the CRT group and the VST group(P=0.962,0.587,0.290,0.581,0.581,0.362).There was also no statistical difference in the the CCLRU grade of corneal fluorescein stain[(78 eyes,16 eyes,8 eyes)and(83 eyes,14 eyes,5 eyes)eyes respectively in Grade 0,Ⅰ,Ⅱ]between the CRT group and the VST group(χ^(2)=0.33,P=0.845).Conclusion After wearing orthokeratology lens for 2 years,the efficacy in myopia control of VST lens is not significantly different from that of CRT lens.
