摘 要:目的分析白内障摘出术中不同类型支撑襻人工晶状体(IOL)对后囊浑浊的影响。方法回顾性队列研究。将郑州大学第一附属医院眼科2021年8月至2022年8月行白内障手术治疗的年龄相关性白内障130例(130眼)纳入研究。患者按术中植入的人工晶状体不同设计分为4组:"L"襻组(34眼)、改良"C"襻组(30眼)、平板襻组(36眼)及四襻组(30眼)。术后随访3个月,以SS-OCT测量IOL光学面与后囊之间的空间面积(AREAP)及前后囊与IOL的贴附情况。以裂隙灯显微镜观察晶状体后囊皱褶及浑浊情况。结果"L"襻组、改良"C"襻组、平板襻组及四襻组患者术后3个月AREAP依次为[0.01(0.00,0.40)]mm^(2)、[0.05(0.02,0.30)]mm^(2)、[0.00(0.00,0.05)]mm^(2)、0,4个组之间差异有统计学意义(F=44.95,P<0.001);前囊与IOL光学面的贴附率依次为47.10%(16/34)、60.00%(18/30)、91.70%(33/36)、100.00%(30/30),4个组间差异有统计学意义(χ^(2)=33.04,P<0.001);后囊的贴附率依次为20.60%(7/34)、50.00%(15/30)、88.90%(32/36)、86.70%(26/30),4个组之间差异有统计学意义(χ^(2)=48.16,P<0.001);后囊皱褶发生率依次为38.20%(13/34)、43.30%(13/30)、8.30%(3/36)、13.30%(4/30),4个组之间差异有统计学意义(χ^(2)=15.89,P=0.001);后囊浑浊发生率依次为32.40%(11/34)、36.70%(11/30)、11.10%(4/36)、6.70%(2/30),4个组之间差异有统计学意义(χ^(2)=12.66,P=0.005)。结论与"L"襻及改良"C"襻IOL相比,平板襻及四襻IOL术后与前后囊的贴附率较高,后囊皱褶及浑浊的发生率较低。Objective To analyse the effect of different haptic intraocular lens(IOL)on posterior capsule opacity after cataract extraction surgery.Methods This was a retrospective cohort study.A total of 130 eyes of 130 cases of age-related cataract who underwent cataract surgery from Aug.2021 to Aug.2022 in the First Affiliated Hospital of Zhengzhou University were included.Patients were divided into 4 groups based on different designs of intraoperative implanted intraocular lens:"L"-haptic group with 34 eyes,modified-"C"-haptic group with 30 eyes,plate-haptic group with 36 eyes and four-haptic group with 30 eyes.At 3 months after operation,the area between IOL and posterior capsule(AREAP)was measured with SS-OCT,and then the adhesion of the anterior and posterior capsule to the IOL was observed.The slit lamp microscope observed the occurrence of posterior capsule wrinkle and opacification.Results At 3 months after surgery,AREAP of the"L"-haptic IOL group,modified-"C"-haptic IOL group,plate-haptic IOL group and four-haptic IOL group were[0.01(0.00,0.40)]mm^(2),[0.05(0.02,0.30)]mm^(2),[0.00(0.00,0.05)]mm^(2)and 0,respectively.The differences were statistically significant among the four groups(F=44.95,P<0.001).The adhesion rates of the anterior capsule to the IOL optical surface were 47.10%(16/34),60.00%(18/30),91.70%(33/36),and 100.00%(30/30),respectively.The differences were statistically significant among the four groups(χ^(2)=33.04,P<0.001).The adhesion rate of the posterior capsule to the IOL optical surfaces were 20.60%(7/34),50.00%(15/30),88.90%(32/36),86.70%(26/30),respectively.The differences were statistically significant among the four groups(χ^(2)=48.16,P<0.001).The incidence rates of posterior capsule wrinkles were 38.20%(13/34),43.30%(13/30),8.30%(3/36),and 13.30%(4/30),respectively.The differences were statistically significant among the four groups(χ^(2)=15.89,P=0.001).The incidence rates of posterior capsule opacity were 32.40%(11/34),36.70%(11/30),11.10%(4/36),and 6.70%(2/30),respectively.The difference
