血清胃蛋白酶原I、胃泌素17、同型半胱氨酸与慢性萎缩性胃炎病情进展及幽门螺杆菌感染的关系

摘　　要：目的:探究血清胃蛋白酶原I(pepsinogen I,PGI)、胃泌素17(gastrin 17,G17)、同型半胱氨酸(homocysteine,Hcy)水平与慢性萎缩性胃炎患者病情进展及幽门螺杆菌(Helicobacter pylori,Hp)感染的相关性。方法:选择武汉市汉阳医院于2021年4月至2022年4月收治的60例慢性萎缩性胃炎患者作为研究对象,比较不同炎症反应严重程度、胃炎评估(operative link for gastritis assessment,OLGA)分期和Hp感染情况中患者的血清PGI、G17、Hcy表达水平。结果:炎症反应严重程度评级、OLGA分期越高,血清PGI、G17水平越低,Hcy水平越高,不同炎症反应严重程度评级、OLGA分期患者的血清PGI、G17和Hcy水平比较,差异均有统计学意义(均P<0.05)。Hp阳性患者血清PGI、G17水平均低于Hp阴性患者,Hcy水平高于Hp阴性患者(均P<0.05)。PGI、G17水平与慢性萎缩性胃炎患者的炎症反应严重程度、OLGA分期和Hp感染呈负相关(均P<0.05),Hcy水平与炎症反应严重程度、OLGA分期和Hp感染呈正相关(均P<0.05)。结论:慢性萎缩性胃炎患者的病情发展和Hp感染与其血清PGI、G17、Hcy水平密切相关,上述指标的变化能够为慢性萎缩性胃炎的早期筛查以及Hp感染的初步预测提供一定参考。Objective: To explore the correlation of serum levels of pepsinogen I (PGI), gastrin 17 (G17), and homocysteine (Hcy) with the progression of chronic atrophic gastritis and Helicobacter pylori (Hp) infection. Methods: A total of 60 patients with chronic atrophic gastritis treated in Wuhan Hanyang Hospital from April 2021 to April 2022 were selected as research subjects. The expression levels of serum PGI, G17, and Hcy in patients with different inflammatory severity, operative link for gastritis assessment (OLGA) stage, and Hp infection were compared. Results: The severity rating and OLGA stage of inflammatory reaction were higher, the levels of serum PGI and G17 were lower, and the level of Hcy was higher, there were significant differences in serum PGI, G17, and Hcy among different severity ratings and OLGA stages (all P<0.05). The levels of serum PGI and G17 in Hp positive patients were lower than those in Hp negative patients, and the levels of Hcy were higher than those in Hp negative patients (all P<0.05). The levels of PGI and G17 were negatively correlated with the severity of inflammatory reaction, OLGA stage and Hp infection in patients with chronic atrophic gastritis (all P<0.05), while the level of Hcy was positively correlated with the severity of inflammatory reaction, OLGA stage, and Hp infection (all P<0.05). Conclusion: The development of chronic atrophic gastritis patients and Hp infection are closely related to their serum PGI, G17, and Hcy. The changes of the above indexes can provide some reference for the early screening of chronic atrophic gastritis and the preliminary prediction of Hp infection.
