激光周边虹膜切开术联合抗VEGF药物治疗急性闭角型青光眼临床前期的效果研究

摘　　要：目的:探讨激光周边虹膜切开术(LPI)联合抗血管内皮生长因子(VEGF)药物治疗急性闭角型青光眼临床前期的效果。方法:回顾性分析2017年11月至2019年7月青岛市第八人民医院眼科中心收治的25例(25眼)急性闭角型青光眼临床前期患者的病历资料,所有患者均行LPI,同时术前1周向玻璃体腔内注射雷珠单抗注射液(0.5 mg/d)。比较患者术前及术后1周的中央前房深度(CACD)、周边前房深度(PACD)、前房容积(ACV)、前房角(ACA)及瞳孔直径(PD)。结果:25例患者术前的PACD分别为上方(0.87±0.14)mm、下方(1.11±0.17)mm、鼻侧(0.91±0.15)mm、颞侧(1.24±0.13)mm,与术后的(1.04±0.14)mm、(1.32±0.17)mm、(1.08±0.16)mm、(1.39±0.16)mm相比差异有统计学意义(P<0.05)。25例患者术前的ACV为(72.89±12.65)mm^(3),与术后的(88.22±14.64)mm^(3)相比差异有统计学意义(P<0.05)。25例患者术前的ACA为(21.58±3.07)mm,与术后的(24.48±2.50)mm相比差异有统计学意义(P<0.05)。25例患者术前的CACD为(1.92±0.19)mm,与术后的(1.95±0.17)mm相比差异无统计学意义(P>0.05)。25例患者术前的PD为(2.65±0.48)mm,与术后的(2.76±0.27)mm相比差异无统计学意义(P>0.05)。结论:LPI联合抗VEGF药物治疗急性闭角型青光眼临床前期可显著增加患者的PACD与ACV,是此病的有效治疗手段之一。Objective:To investigate the effect of laser peripheral iridotomy(LPI)combined with anti-vascular endothelial growth factor(VEGF)drugs in the treatment of acute angleclosure glaucoma in the early clinical stage.Methods:The medical records of 25 patients(25 eyes)with preclinical acute angle-closure glaucoma admitted to the Ophthalmic Center of the Eighth People’s Hospital of Qingdao from November 2017 to July 2019 were retrospectively analyzed.All patients were treated with LPI and injected with Leizumab injection(0.5 mg/d)into the vitreal cavity 1 week before surgery.The central anterior chamber depth(CACD),peripheral anterior chamber depth(PACD),anterior chamber volume(ACV),anterior chamber Angle(ACA)and pupil diameter(PD)were compared before and one week after surgery.Result:The preoperative PACD of 25 patients were above(0.87±0.14)mm,below(1.11±0.17)mm,nasal(0.91±0.15)mm,temporal(1.24±0.13)mm,respectively.The difference was statistically significant compared with(1.04±0.14)mm,(1.32±0.17)mm,(1.08±0.16)mm,(1.39±0.16)mm after operation(P<0.05).The ACV of 25 patients before surgery was(72.89±12.65)mm^(3),and the difference was statistically significant compared with that after surgery(88.22±14.64)mm^(3)(P<0.05).The ACA of the 25 patients before surgery was(21.58±3.07)mm,and the difference was statistically significant compared with that after surgery(24.48±2.50)mm(P<0.05).The preoperative CACD of the 25 patients was(1.92±0.19)mm,and there was no significant difference between the preoperative CACD and the postoperative CACD of the 25 patients(1.95±0.17)mm(P>0.05).The PD of the 25 patients before surgery was(2.65±0.48)mm,and there was no significant difference between the PD of the 25 patients and that of the 25 patients after surgery(2.76±0.27)mm(P>0.05).Conclusion:LPI combined with anti-VEGF drugs can significantly increase the PACD and ACV of patients with acute angleclosure glaucoma in the early clinical stage,which is one of the effective treatments for this disease.
