摘　　要：目的:探讨SPECT全身骨显像联合肿瘤标志物检测在老年肺癌骨转移诊断中的应用价值。方法:选择2018年1月至2022年8月作为研究时段,将该时段内在中山陈星海中西医结合医院接受治疗的34名出现骨转移的肺癌患者作为骨转移组。选取30名未出现骨转移的肺癌患者作为对照组,比较两组研究对象的SPECT全身骨显像联合肿瘤标志物检测结果。结果:与对照组患者相比,骨转移组患者的各项血清肿瘤标志物水平均更高,差异有统计学意义(P<0.05)。与对照组患者相比,骨转移组患者CYFRA 21-1、CEA、CA125、NSE的阳性率均更高,差异有统计学意义(P<0.05)。SPECT全身骨显像与常规CT诊断骨转移的准确率分别为94.12%、79.41%。与常规CT相比,SPECT全身骨显像诊断骨转移的准确率更高,差异有统计学意义(P<0.05)。结论:SPECT全身骨显像联合肿瘤标志物检测在老年肺癌骨转移诊断中的应用价值较高,有助于医务人员了解患者的病情,使患者的后续治疗工作更为顺利地开展。此法值得在临床上推广应用。Objective:To investigate the application value of SPECT combined with tumor markers in the diagnosis of bone metastases in elderly lung cancer.Methods:January 2018 to August 2022 were selected as the study period,and 34 lung cancer patients with bone metastasis who received treatment in Zhongshan Chenxinghai Hospital of Integrated Traditional Chinese and Western Medicine during this period were selected as the bone metastasis group.Thirty patients with lung cancer without bone metastasis were selected as the control group,and the results of whole body bone imaging combined with tumor markers were compared between the two groups.Results:Compared with control group,the levels of serum tumor markers in bone metastasis group were higher,and the differences were statistically significant(P<0.05).Compared with control group,the positive rates of CYFRA 21-1,CEA,CA125 and NSE in bone metastasis group were higher,with statistical significance(P<0.05).The accuracy of whole body bone imaging and conventional CT in diagnosing bone metastasis was 94.12%and 79.41%respectively.Compared with conventional CT,SPECT was more accurate in diagnosing bone metastases,and the difference was statistically significant(P<0.05).Conclusion:SPECT combined with tumor marker detection has high application value in the diagnosis of bone metastases of elderly lung cancer,which is helpful for medical staff to understand the condition of patients and make the follow-up treatment of patients more smoothly.This method is worth popularizing in clinic.
