理中汤合半夏厚朴汤加减治疗胃食管反流病(寒证型)疗效分析

摘　　要：目的探讨理中汤合半夏厚朴汤加减治疗在胃食管反流病(GERD)(寒证型)患者中的临床作用。方法GERD(寒证型)患者100例,随机分为中药治疗组及西药对照组,比较2组治疗前后临床疗效,中医症状评分,不良反应,胃镜下食管黏膜炎症改善情况及复发率。结果中药治疗组患者总有效率为82.0%,显著优于西药对照组的60.0%,差异有统计学意义(P<0.05);治疗后,2组中医症状评分均较治疗前减少,且中医治疗组患者在治疗后各症状积分均较西药对照组下降更明显(P<0.05);中药治疗组患者胃镜下食管黏膜炎症改善总有效率为72.0%,显著高于西药对照组的50.0%,差异有统计学意义(P<0.05);中药治疗组患者的复发率(12.0%)明显低于西药对照组(52.0%)(P<0.05)。结论理中汤并半夏厚朴汤加减治疗在GERD(寒证型)中的应用,可明显改善患者的临床症状及胃镜下表现并降低复发率。Objective To observe the clinical effect of Lizhong Decoction combined with modified Banxia Houpo Decoction on gastroesophageal reflux disease(GERD)(cold syndrome).Methods A total of 100 patients with GERD(cold syndrome)were randomly divided into observation group and control group,and they were managed by traditional Chinese medicine(TCM)treatment and conventional Western medicine,respectively.The clinical efficacy,TCM syndrome scores,adverse reactions,improvement of esophageal mucosal inflammation under the gastroscope and recurrence rate were compared between the two groups before and after treatment.Results The total effective rate in the observation group was significantly higher than that in the control group(82.0%vs 60.0%,P<0.05).After treatment,the TCM symptom scores in the both groups decreased compared with those before treatment,which were more pronounced in the observation group than those in the control group(P<0.05).The total effective rate of the improvement of esophageal mucosal inflammation under the gastroscope in the observation group was significantly higher than that in the control group(72.0%vs 50.0%,P<0.05).The recurrence rate in the observation group was significantly lower than that in the control group(12.0%vs 52.0%,P<0.05).Conclusion The application of Lizhong Decoction combined with modified Banxia Houpu Decoction can significantly improve the clinical symptoms and gastroscopic performance and reduce the recurrence rate in patients with GERD(cold syndrome).
