摘　　要：目的研究快速康复执行单在肺纵隔手术中的应用,对患者术后康复指标的影响、术后疼痛评分(VAS)的变化、并发症的发生率以及患者肺功能指标用力呼气容积(FEV1)的恢复及生活质量的影响效果。方法随机为筛选2020年1月至2021年1月胸外科接受肺纵隔手术的患者100例为研究对象,随机分成观察组与对照组,每组50例。对照组患者围手术期采取胸外科常规护理干预;观察组围手术期应用快速康复执行单对患者的饮食、运动、功能锻炼、健康教育、疼痛、睡眠等进行护理干预。将两种护理干预模式下术后并发症发生率、术后不同时间段VAS评分,术前、术后肺功能指标用力呼气容积(FEV1)等指标进行对比。结果观察组术后并发症发生率较对照组降低(P<0.05);观察组首次下床时间、首次排气时间、首次排便时间、胸管留置时间比对照组均缩短(P<0.05);观察组术后6 h、24 h、48 h、72 h VAS评分均低于对照组(P<0.05);观察组患者术后FEV1较对照组改善明显,生活质量较对照组提高,术后住院时间较对照组缩短(P<0.05);观察组护理满意度较对照组提高(P<0.05)。结论快速康复执行单在肺纵隔手术中的应用及护理干预,取得的临床治疗效果较好,提高了患者的生活质量和满意度。Objective To investigate the application effect of rapid rehabilitation exercise sheet in pulmonary mediastinal surgery,and its influence on postoperative rehabilitation index,visual analog scale(VAS)for pain,complication incidence,forced expiratory volume in one second(FEV1),quality of life.Methods From January 2020 to January 2021,one-hundred patients who underwent pulmonary mediastinal surgery in the Department of Thoracic Surgery were randomly assigned into the observation group(n=50)and the control group(n=50).On the basis of perioperative conventional nursing intervention,the observation group received nursing intervention including diet,exercise,functional exercise,health education,pain,sleep.The aim was to compare pre and post-operative complication,VAS,FEV1.Results After treatment,the postoperative complication in the observation group was decreased than that in the control group(P<0.05).The first time of out of bed,first exhaust time,first defecation time,chest tube indwelling time,postoperative length of stay in the observation group were shorter than those in the control group(all P<0.05).VAS scores at 6,24,48,72 hours postoperatively were decreased(P<0.05),FEV1,quality of life,nursing satisfactory degree were significantly improved(all P<0.05).Conclusion For pulmonary mediastinal surgery,the rapid rehabilitation exercise sheet has good clinical effect,which improves the quality of life and satisfaction.
