摘  要:目的探讨血清可溶性神经纤毛蛋白-1(NRP-1)、肉碱棕榈酰转移酶1A(CPT1A)、网膜素-1(Omentin-1)水平与乳腺癌患者临床特征及预后的关系。方法选取自2017年1月至2018年12月收治的乳腺癌患者135例(乳腺癌组)。另选同期的健康体检者100例(健康组)。检测各组血清sNRP-1、CPT1A、Omentin-1水平。随访2年,详细记录患者的预后状况,并应用多因素Logistics回归分析影响患者临床预后的相关因素。结果乳腺癌组血清sNRP-1、CPT1A显著高于健康组,Omentin-1水平低于健康组,差异均有统计学意义(P<0.05)。不同肿瘤分化程度、淋巴结转移情况及临床分期的乳腺癌患者,血清sNRP-1、CPT1A、Omentin-1水平比较,差异均有统计学意义(P<0.05)。乳腺癌患者预后不良组血清sNRP-1、CPT1A水平高于预后良好组,血清Omentin-1水平低于预后良好组,差异有统计学意义(P<0.05)。多因素分析结果显示,肿瘤低分化、淋巴结转移、临床分期Ⅲ期、血清sNRP-1和CPT1A升高、血清Omentin-1降低是影响乳腺癌患者不良预后的独立危险因素(P<0.05)。结论乳腺癌患者血清sNRP-1、CPT1A、Omentin-1水平与患者临床病理特征和预后不良有关,肿瘤低分化、淋巴结转移、临床分期Ⅲ期、血清sNRP-1和CPT1A升高、血清Omentin-1降低是影响乳腺癌患者不良预后的独立危险因素。早期联合检测血清sNRP-1、CPT1A、Omentin-1对判断患者的生存预后有较高的临床价值。Objective To explore the relationship between serum soluble neuropilin-1(NRP-1),carnitine palmitoyl transferase 1A(CPT1A),omentin-1(Omentin-1)levels and the clinical characteristics and prognosis of breast cancer patients.Methods A total of 135 breast cancer patients(breast cancer group)admitted to our department from January 2017 to December 2018 were selected.During the same period,another 100 healthy subjects(healthy group)who were in our hospital during this period were selected.Serum levels of sNRP-1,CPT1A and Omentin-1 in each group were detected.Followed up for 2 years,the prognostic status of the patients was recorded in detail,and the multivariate logistic regression was used to analyze the relevant factors affecting the clinical prognosis of patients.Results The levels of sNRP-1 and CPT1A in breast cancer group were significantly higher than those in healthy group,and the levels of Omentin-1 were lower than those in healthy group,with statistical significance(P<0.05).There were significant differences in serum levels of sNRP-1,CPT1A and Omentin-1 among breast cancer patients with different degrees of tumor differentiation,lymph node metastasis and clinical stage(P<0.05).The serum levels of sNRP-1 and CPT1A in the poor prognosis group were higher than those in the good prognosis group,and the serum Omentin-1 level was lower than that in the good prognosis group,with statistical significance(P<0.05).Multivariate analysis showed that low tumor differentiation,lymph node metastasis,clinical stageⅢ,increased serum sNRP-1 and CPT1A,and decreased serum Omentin-1 were independent risk factors for poor prognosis of breast cancer patients(P<0.05).Conclusion Serum levels of sNRP-1,CPT1A and Omentin-1 in breast cancer patients are associated with clinicopathological features and poor prognosis of patients.Low tumor differentiation,lymph node metastasis,clinical stageⅢ,elevated serum sNRP-1 and CPT1A,and decreased serum Omentin-1 are independent risk factors for poor prognosis of breast cancer patients.Early com
