溃结方治疗难治性溃疡性结肠炎疗效研究

摘　　要：目的探讨溃结方治疗难治性溃疡性结肠炎的临床疗效。方法选取自2020年1月至2022年12月北部战区总医院收治的199例难治性溃疡性结肠炎患者为研究对象。所有患者均口服中药溃结方或联合灌肠,治疗3个月后,对患者的中医证候和Baron结肠黏膜溃疡进行评分。结果治疗3个月后,总有效率为94.5%(188/199)。治疗3个月后,199例患者腹泻、腹痛、脓血便、里急后重的中医症状积分分别为(0.41±0.82)分、(0.24±0.43)分、(0.39±0.92)分、(0.06±0.24)分,明显低于治疗前的(3.16±1.92)分、(1.46±1.57)分、(3.08±1.94)分、(0.74±0.89)分,差异均有统计学意义(P<0.05)。治疗3个月后,199例患者Baron结肠镜评分为(0.74±0.64)分,明显低于治疗前的(2.29±0.88)分,差异有统计学意义(P<0.05)。结论口服中药溃结方或联合应用灌肠治疗难治性溃疡性结肠炎的临床效果确切,腹泻、腹痛、黏液脓血便及里急后重等症状改善明显,肠黏膜的炎症损伤缓解明显。Objective To investigate the curative effect of Kuijie prescription in the treatment of refractory ulcerative colitis.Methods A total of 199 patients with refractory ulcerative colitis admitted from January 2020 to December 2022 were selected as subjects.All the patients received Kuijie prescription or combined with retention-enema.The course of treatment was 3 months.The traditional Chinese medicine(TCM)syndrome scores and Baron colon mucosal ulcer of patients before and after treatment were observed to evaluate the clinical efficacy.Results After 3 months of treatment,the total effective rate was 94.5%(188/199).After 3 months of treatment,the scores of TCM symptoms of diarrhea,abdominal pain,abscess and blood stool,and posterior tenesmus of 199 patients were(0.41±0.82)points,(0.24±0.43)points,(0.39±0.92)points,(0.06±0.24)points,respectively.It was significantly lower than that before treatment(3.16±1.92)points,(1.46±1.57)points,(3.08±1.94)points,(0.74±0.89)points,the differences were statistically significant(P<0.05).The score of Baron colon mucosal ulcer was(0.74±0.64)points,which was significantly lower than that before treatment(2.29±0.88)points,and the difference was statistically significant(P<0.05).Conclusion Kuijie prescription or combined with retention-enema is a feasible treatment of refractory ulcerative colitis,since it can significantly relieve the symptoms of diarrhea,abdominal pain,abscess and blood stool and posterior tenesmus,and can significantly relieve the inflammatory damage of intestinal mucosa.
