宫颈活检联合阴道镜检查在宫颈癌早期筛查中的作用分析

摘　　要：目的探讨宫颈活检联合阴道镜检查在宫颈癌早期筛查中的应用价值。方法选择250例液基薄层细胞学检查(TCT)异常患者,均行碘试验下宫颈活检及阴道镜检查,以术后病理及随访结果为金标准,分析宫颈活检、宫颈活检联合阴道镜检查的诊断符合率及与金标准结果的一致性,并比较两种方法的诊断效能。结果以术后病理及随访结果为金标准,宫颈活检的诊断符合率为88.40%,与金标准结果的一致性一般(Kappa=0.59);宫颈活检联合阴道镜检查的诊断符合率为96.80%,与金标准结果的一致性较好(Kappa=0.87);宫颈活检联合阴道镜检查的诊断准确率为97.20%、敏感度为97.06%、特异度为97.22%,均高于宫颈活检的89.20%、79.41%、90.74%(P<0.05)。结论宫颈活检联合阴道镜检查在宫颈癌早期筛查中的应用价值较高,相较于宫颈活检,宫颈活检联合阴道镜检查与金标准结果的一致性更好且诊断效能更高。Objective To investigate the application value of cervical biopsy combined with colposcopy in the early screening of cervical cancer.Methods A total of 250 patients with abnormal TCT were selected to undergo cervical biopsy under iodine test and colposcopy were performed.The results of postoperative pathology and follow-up were used as the gold standard to analyze the diagnostic coincidence rate of cervical biopsy and cervical biopsy combined with colposcopy and the consistency with the gold standard results,and to compare the diagnostic eficacy of the two methods.Results With postoperative pathology and follow-up as the gold standard,The diagnosis coincidence rate of cervical biopsy was 88.40%,which was generally consistent with the results of gold standard(Kappa=0.59).The coincidence rate of cervical biopsy combined with colposcopy was 96.80%,which was in good agreement with the results of gold standard(Kappa=0.87).The diagnostic accuracy,sensitivity and specificity of cervical biopsy combined with colposcopy were 97.20%,97.06%and 97.22%,all higher than those of cervical biopsy(89.20%,79.41%and 90.74%,P<0.05).Conclusion Cervical biopsy combined colposcopy has high application value in the early screening of cervical cancer.Compared with cervical biopsy,cervical biopsy combined with colposcopy has better consistency with the gold standard results and has higher diagnostic efficacy.
