基于图论与fMRI探究太极拳对慢性疲劳综合征患者默认模式网络的影响

摘　　要：目的:从默认模式网络(DMN)探讨太极拳缓解慢性疲劳综合征(CFS)的机制。方法:招募20例CFS患者和20名健康人进行每周7次,每次30 min,持续4周的太极拳训练,并进行功能磁共振成像技术(fMRI)扫描、疲劳量表(FS-14)以及健康调查简表(SF-36)评估,比较两组治疗前后DMN拓扑属性值和临床量表的变化。结果:太极拳训练前后,CFS组的FS-14评分显著降低(P<0.001),SF-36评分显著升高(P<0.001),全局效率显著降低(P=0.010),其中双侧前额叶皮质和右侧后扣带回皮质的节点效率降低尤为明显(P<0.00055)。结论:太极拳可能通过降低双侧前额叶皮质和右侧后扣带回皮质功能活动,整体降低DMN的全局网络效率,达到缓解CFS患者疲劳感、促进其生活健康状态的临床疗效。Objective:To explore the mechanism of Tai Chi to alleviate chronic fatigue syndrome(CFS)from the default mode network(DMN).Methods:Twenty patients with CFS and 20 healthy individuals were recruited to undergo Tai Chi training for 30 minutes seven times a week for 4 weeks,and were assessed with fMRI scans,the Fatigue Scale(FS-14),and the Health Survey Short Form(SF-36)to compare changes in DMN's topological attributions and clinical scales before and after treatment in both groups.Results:Compared to healthy individuals,Taichiquan training resulted in significantly lower FS-14 scores(P<0.001),significantly higher SF-36 scores(P<0.001),and significantly lower global efficiency(P=0.010)in the CFS group,with particularly significant reductions in nodal efficiency in bilateral prefrontal cortex and right posterior cingulate cortex(P<0.00055).Conclusion:Tai Chi may reduce the global network efficiency of DMN overall by decreasing the functional activity of bilateral prefrontal cortex and right posterior cingulate cortex,achieving the clinical effcacy of relieving fatigue and promoting a healthy stateof life inCFS patients.
