玻璃体切除联合C3F8填充术治疗糖尿病视网膜病变的临床疗效观察

摘　　要：目的探讨玻璃体切除联合C3F8填充术治疗糖尿病视网膜病变(DR)的临床疗效。方法收集2020年1月至2022年6月宁波大学附属人民医院收治的59例(61眼)早中期增殖性视网膜病变(PDR)患者,按随机数字表法分为单纯玻璃体切除组(对照组X 32例33眼)和玻璃体切除联合C3F8填充组(观察组X 27例28眼)。比较两组手术前后视力、眼压、黄斑中心凹厚度变化及术后视网膜脱离、白内障发生率与局部视网膜牵拉复位情况。结果两组术后视力均较术前提高(P<0.05),而眼压与黄斑中心凹厚度均较术前降低(均P<0.05),但两组术后视力、眼压、黄斑中心凹厚度、视网膜脱离及白内障发生率差异均无统计学意义(均P>0.05)。观察组局部视网膜牵拉复位情况明显优于对照组。结论玻璃体切除联合C3F8填充术治疗能有效提高早中期PDR患者的视力,降低黄斑中心凹厚度,使局部牵拉的视网膜有效复位。Objective To investigate the clinical efficacy of vitrectomy combined with C3F8 tamponade for the treatment of diabetic retinopathy.Methods Fifty-nine patients(61 eyes)with early to mid-stage proliferative dia-betic retinopathy were selected and divided into vitrectomy-only group(32 cases with 33 eyes)and vitrectomy com-bined with C3F8 tamponade group(27 cases with 28 eyes)according to the random number table method.Visual acu-ity,intraocular pressure(IOP),macular central recess thickness,the incidence of postoperative retinal detachment,cataract,and local retinal traction repositioning were investigated before and after surgery in both groups.Results Patients in both groups had significantly better visual acuity after surgery(allP<0.05),while IOP and macular cen-tral recess thickness were both lower than preoperative(allP<0.05);however,there were no significant differences.between two groups(all P>0.05).Both groups had similar incidence of postoperative retinal detachment and cata-ract,while combined group had significantly better local retinal retraction and repositioning than vitrectomy-only gro-up(all P<0.05).Conclusions Vitrectomy combined with C3F8 flling treatment can effectively improve visual acuity,reduce macular central recess thickness,and restore the local retinal retinal retraction effectively in patients with early to mid-stage proliferative diabetic retinopathy.
