角膜基质环植入术后角膜融解1例  

Corneal melt after intracorneal ring segment implantations:a case report

摘　　要：患者为50岁女性,因双眼角膜基质环植入术后18年右眼疼、流泪、睁眼困难1个月余就诊。裂隙灯显微镜检查发现右眼角膜基质环边缘3~4点钟方位处存在角膜融解变薄和荧光素渗漏现象;相干光层析成像术提示右眼3~4点钟方位角膜基质环前表面上皮层及浅基质层缺失。根据临床表现及检查结果,诊断为右眼角膜基质环植入术后角膜融解。随后在全身麻醉下实施了右眼角膜基质环取出联合深板层角膜移植术。术后患者无不适症状,角膜植片透明度良好。A 50-year-old female patient presented to the Beijing Tongren Eye Center,Beijing Tongren Hospital,Capital Medical University,Beijing Institute of Ophthalmology,with complaints of right eye pain,tearing,and difficulty opening the eye for over a month after intrastromal corneal ring segment(ICRS)implantation 18 years prior in both eyes.Slit lamp examination revealed corneal stromal melting around the ICRS at the 3 to 4 o′clock position of the right eye,with fluorescein staining.Optical coherence tomography showed epithelial and superficial stromal layer defects in the area of the lesion.The patient was diagnosed with corneal melting after ICRS implantation in the right eye.Under general anesthesia,the corneal stromal ring was removed,and deep lamellar keratoplasty was performed.The patient had no discomfort and the corneal graft remained transparent after the surgery.
