膀胱软镜镜检术中改良"Bag Squeeze"法的应用与疼痛焦虑影响因素分析

摘　　要：目的探讨改良"Bag Squeeze"法在膀胱软镜镜检中的临床有效性,分析影响镜检疼痛与焦虑的相关因素。方法回顾性收集我院2020年3月至2021年4月,200例行膀胱软镜镜检男性患者的临床资料,按镜检时是否使用改良"Bag Squeeze"法分为改良"Bag Squeeze"组和对照组,各100例。使用视觉模拟疼痛评分量表(VAS)对患者镜检结束时、结束后24 h的疼痛程度进行评分;依据阿姆斯特丹焦虑和信息评分表(APAIS)对镜检结束时的焦虑情况予以评分。分析两组镜检的疼痛与焦虑评分,比较其组间的差异情况。结果两组患者的年龄、体质量指数、TURBT手术史和既往病史等比较差异均无统计学意义(P>0.05)。改良"Bag Squeeze"组患者在检查结束时、检查后24 h的VAS评分和检查结束时的APAIS评分均显著低于对照组,且差异具有统计学意义(P<0.05)。前列腺增生对检查后疼痛及焦虑均无明显影响,但合并重度前列腺增生可加重患者镜检疼痛。结论改良"Bag Squeeze"法有助于在膀胱软镜镜检中减轻患者疼痛和焦虑,保证检查顺利进行;对于如何缓解重度前列腺增生患者的镜检不适感,仍需进一步探讨。Objective To explore the effects of the modified"Bag Squeeze"technique,and the relative factors of pain and anxiety during flexible cystoscopy.Methods A total of 200 male patients underwent flexible cystoscopy between March 2020 and April 2021.Among them,100 patients received the modified"Bag Squeeze"technique(Group B),while the rest received the non-modified"Bag Squeeze"technique(Group A).All participants completed a pain questionnaire(visual analog scale)and anxious questionnaire(the Amsterdam preoperative anxiety and information scale)after cystoscopy,followed by a repeat pain questionnaire 24 h later.The perioperative outcomes were compared between the two groups.Results Patients in Group A and Group B were similar in terms of age,BMI,previous TUR-bt history,BPH,hypertension,diabetes mellitus,and location.The VAS score at the end of examination,24h after examination and APAIS score at the end of examination in the modified"Bag Squeeze"group were significantly lower than those in the control group,and the differences were statistically significant(P<0.05);benign prostatic hyperplasia had no significant effect on the pain and anxiety after examination,but combined with severe benign prostatic hyperplasia could aggravate the microscopic pain of patients.Conclusions The modified"Bag Squeeze"method helps to reduce the pain and anxiety of patients during flexible cystoscopy and ensure the smooth progress of the examination;how to relieve the discomfort of microscopic examination in patients with severe benign prostatic hyperplasia still needs to be further explored.
