大气下陷之病因探析

摘　　要：通过分析张锡纯《医学衷中参西录》中的医案,探讨大气下陷之病因。认为大气下陷的病因可归为内伤、外感及其他因素。大气下陷以气虚为基础,日久则致陷。元气不足、脾胃损伤或胸中清气匮乏可致大气生成减少,过度劳作、过多宣讲或温热之邪久留体内可使大气消耗过多,而受惊、易怒等情志异常导致人体气机升降失调,亦可影响胸中大气,最终发展为大气下陷之证。临床若见胸闷气喘、短气乏力、心中怔忡、神昏健忘等病症,需仔细询问其病因,可考虑辨为大气下陷,以升陷汤治疗。探讨大气下陷之病因,可为临床诊疗大气下陷证提供思路。This article discussed the etiology of pectoral qi subsidence through analyzing the medical cases recorded in Yi Xue Zhong Zhong Can Xi Lu(Records of Traditional Chinese and Western Medicine in Combination)written by ZHANG Xi-Chun.ZHANG Xi-Chun believes that the etiology of pectoral qi subsidence can be classified into internal injuries,external contraction and other factors.It is believed that pectoral qi subsidence develops from qi deficiency,and then leads to subsidence over time.Insufficient original qi,injury of spleen and stomach,or lack of clear qi in the chest may cause the reduction of the production of pectoral qi;overwork,excessive talking or the long-time retention of warm febrile pathogen in the body may cause excessive consumption of pectoral qi;disordered emotions such as fright and irritability may lead to an imbalance in the ascending and descending of qi movement,which may also affect the pectoral qi in the chest and eventually develop into the syndrome of pectoral qi subsidence.For the patients with the clinical symptoms of chest distress and dyspnea,shortness of breath and fatigue,palpitation,absent-mindedness and forgetfulness,detailed enquiry should be made for the etiology of the symptoms,and the symptoms can be differentiated as pectoral qi subsidence which can be treated with Shengxian Decoction.The exploration of the etiology will provide reference for the clinical diagnosis and treatment of pectoral qi subsidence.
