摘 要:目的研究益肾蠲痹丸对胶原诱导性关节炎小鼠(CIA)血清炎症因子和肠道菌群的影响,从肠道微生态角度探讨其治疗类风湿关节炎的作用机制。方法根据随机数字表法将18只BALB/c小鼠分为空白组、模型组、益肾蠲痹组,每组6只。模型组和益肾蠲痹组小鼠采用尾部注射鸡Ⅱ型胶原和弗氏完全佐剂混合液方法构建CIA模型,第28天造模成功后,益肾蠲痹组开始给予益肾蠲痹丸浓缩剂灌胃,空白组和模型组给予等体积的生理盐水灌胃,均连续灌胃4周。记录3组小鼠造模第28天和灌胃2周、4周后的体重及关节炎指数评分;灌胃4周后,ELISA法检测血清白细胞介素-10(IL-10)、白细胞介素-17A(IL-17A)水平,HE染色观察关节组织病理学形态,16SrDNA法进行肠道菌群分析。结果灌胃2周和4周后,模型组小鼠体重均明显低于空白组(P均<0.05),而益肾蠲痹丸组小鼠体重均明显高于同期模型组(P均<0.05)。灌胃2周后,模型组和益肾蠲痹组小鼠关节炎指数评分比较差异无统计学意义(P>0.05);灌胃4周后,益肾蠲痹组小鼠关节炎指数评分明显低于模型组(P<0.05)。灌胃4周后,模型组小鼠血清IL-10水平明显低于空白组(P<0.05),血清IL-17A水平明显高于空白组(P<0.05),关节腔内见滑膜细胞增生和血管翳生成,肠道菌群丰度和多样性显著下降,菌群结构也发生明显变化;与模型组比较,益肾蠲痹组小鼠血清IL-10水平明显升高(P<0.05),IL-17A水平明显降低(P<0.05),关节腔内无滑膜细胞增生和血管翳,肠道菌群丰度和均匀度均明显增加,且物种差异明显。结论益肾蠲痹丸对CIA小鼠具有显著抗炎和调节肠道内菌群的丰富度和多样性作用,推测其调节肠道内微生态的作用机制可能与调节Th17/Treg的平衡相关。Objective It is to investigate the effects of pills for replenishing kidney and eliminating impediment on serum inflammatory factors and intestinal flora in mice with collagen-induced arthritis(CIA),and to explore its mechanism in the treatment of rheumatoid arthritis from the perspective of intestinal microecology.Methods Eighteen BALB/c mice were divided into blank group,model group and replenishing kidney and eliminating impediment group according to the random number table method,with 6 mice in each group.The CIA models were established by tail injection of mixture liquid of chicken type II collagen and Freund’s complete adjuvant in the mice of the model group and replenishing kidney and eliminating impediment group.After successful modeling on the 28th day,the mice in the replenishing kidney and eliminating impediment group were given the concentrated agent of pills for replenishing kidney and eliminating impediment by gavage,and the blank group and the model group were given the same volume of normal saline by gavage,all continuously treated for 4 weeks.The body weight and arthritis index scores of the mice were recorded on the 28th day of modeling and after 2 and 4 weeks of gavage;after 4 weeks of gavage,the levels of serum interleukin-10(IL-10)and interleukin-17A(IL-17A)were measured by ELISA,joint histopathological morphology was observed by HE staining,and intestinal flora was analyzed by 16SrDNA.Results After 2 and 4 weeks of gavage,the body weight of the mice in the model group was significantly lower than that of the blank group(P<0.05),while the body weight of mice in the replenishing kidney and eliminating impediment group was significantly higher than that of the model group at the same time(all P<0.05).After 2 weeks of gavage,there was no statistically significant difference in the arthritis index scores of mice between the model group and replenishing kidney and eliminating impediment group(P>0.05);after 4 weeks of gavage,the arthritis index scores of mice in the replenishing kidney and elimin
