柴胡桂枝干姜汤治疗胆热脾寒型非酒精性脂肪性肝炎的临床观察

摘　　要：目的:观察柴胡桂枝干姜汤治疗胆热脾寒型非酒精性脂肪性肝炎(NASH)的临床疗效。方法:将90例胆热脾寒型NASH患者随机分为对照组和观察组,每组45例。对照组患者口服多烯磷脂酰胆碱胶囊,观察组患者在对照组患者治疗的基础上口服柴胡桂枝干姜汤配方颗粒,两组患者均治疗12周。观察比较两组患者治疗前后的中医证候积分、生化指标、肝脏脂肪含量值。结果:治疗后,观察组患者的中医证候积分有效率优于对照组患者(P<0.05)。治疗后,两组患者的谷丙转氨酶(ALT)、总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白(HDL-C)值与治疗前比较,差异有统计学意义(P<0.05);治疗后,观察组患者的TC、TG值低于对照组患者(P<0.05)。治疗后,观察组患者的肝脏脂肪含量检测疗效有效率高于对照组(P<0.05)。结论:柴胡桂枝干姜汤治疗胆热脾寒型NASH患者,能有效改善患者中医证候积分和生化指标,降低患者的肝脏脂肪含量。Objective:To observe the clinical curative effect of Chaihu Guizhi Ganjiang Decoction on nonalcoholic steatohepatitis(NASH)with gallbladder heat and spleen cold.Methods:A total of 90 patients with NASH of gallbladder heat and spleen cold type were randomly divided into control group and observation group,with 45 cases in each group.The control group was treated with polyene phosphatidylcholine capsules,while the observation group was treated with Chaihu Guizhi Ganjiang Decoction formula granules on the basis of the control group.Both groups were treated for 12 weeks.The TCM symptom scores,biochemical indexes and liver fat content of the two groups were compared before and after treatment.Results:After treatment,the effective rate of TCM symptoms in the observation group was better than that in the control group(P<0.05).After treatment,the values of alanine aminotransferase(ALT),total cholesterol(TC),triglyceride(TG)and high density lipoprotein(HDL-C)in the two groups were significantly different from those before treatment(P<0.05).After treatment,the TC and TG values of patients in the observation group were lower than those in the control group(P<0.05).After treatment,the effective rate of liver fat content in the observation group was higher than that in the control group(P<0.05).Conclusion:Chaihu Guizhi Ganjiang Decoction can effectively improve the TCM symptoms and biochemical indexes of patients with NASH due to gallbladder heat and spleen cold,and reduce the liver fat content of patients.
