摘　　要：伴骨和软骨分化的转移性黑色素瘤极为罕见,易误诊为肉瘤。本文报告一例42岁男性患者,因左足拇趾甲癣病史3年,甲板剥脱1年,甲床增生、渗血半年于2017年12月进行左足拇趾甲床清创术,病理诊断为恶性黑色素瘤。随后行左足拇趾截趾术、大剂量干扰素和PD-1免疫治疗,但效果欠佳,多次转移,其中腹股沟结节经组织病理学诊断为转移性黑色素瘤伴骨和软骨分化,最终患者死亡。Metastatic melanoma with osteo-cartilaginous differentiation is extremely rare and can be misdiagnosed as sarcoma.A 42-year-old male patient referred to clinic with left foot great toe onychomycosis for 3 years,nail plate stripping for 1 year,nail bed hyperplasia and blooding for half one year,had gone the debridement of the left foot hallux nail bed at Dec 2017.The patient was pathologically diagnosed with malignant melanoma,which was poorly treated by left foot hallux amputation,high-dose interferon and PD-1 immunotherapy,after multiple metastases,in which the inguinal nodule was pathologically diagnosed as metastatic malignant melanoma with osteo-cartilaginous differentiation,and then died.
