老年脑膜瘤病人术前睡眠状况调查及其影响因素分析

摘　　要：目的探讨老年脑膜瘤病人术前睡眠质量及其影响因素。方法选取空军军医大学第一附属医院神经外科三病区2020年1~12月间收治的老年(≥65岁)脑膜瘤病人71例,按照病人手术前匹兹堡睡眠质量指数量表(PSQI)评分判断睡眠质量。采用单因素分析筛选影响老年脑膜瘤病人睡眠障碍(SD)的相关因素,并采用多因素Logistic回归分析明确其独立危险因素。结果71例老年脑膜瘤病人中,SD组34例(47.9%),睡眠良好组37例(52.1%)。单因素分析显示,2组病人BMI、锻炼情况、数字疼痛评分量表(NRS)评分、卡式评分(KPS)、多维度疲劳量表(MFI-20)评分、医院用焦虑抑郁量表(HADS-A、HADS-D)评分比较,差异具有统计学意义(P<0.05或P<0.01)。多因素Logistic回归分析显示,高NRS、HADS-A、MFI-20评分以及低KPS评分是影响老年脑膜瘤病人SD发生的独立危险因素。结论SD是老年脑膜瘤病人的常见并发症状之一,影响SD的独立危险因素包括身体功能状态、焦虑、疼痛及疲劳等。Objective To study the prevalence and risk factors of sleep quality in the elderly patients with meningiomas before surgery.Methods Seventy-one elderly patients with meningiomas aged over 65 years old admitted to Neurosurgery Department 3rd Ward of the First Affiliated Hospital,Air Force Military Medical University from January to December 2020 were retrospectively analyzed.The Pittsburgh Sleep Quality Index(PSQI)was used to assess the quality of sleep.Univariate analysis and multivariate Logistic regression were used to identify the independent risk factors of sleep disturbance(SD).Results Among the 71 patients,34(47.9%)suffered from SD and 37(52.1%)showed good sleep quality.Univariate analysis showed that body mass index(BMI),exercise,the scores of Numerical Rating Scale(NRS),Karnofsky Performance Status(KPS),Multidimensional Fatigue Inventory(MFI-20)and Hospital Anxiety and Depression Scale(HADS)were significantly different between the PSQI-based subgroups(P<0.05 or P<0.01).Multivariate Logistic regression model further identified that higher NRS score,higher MFI-20 score,higher HADS-A score and lower KPS score were the independent risk factors for SD in the elderly patients with meningiomas.Conclusions SD is one of the common symptoms in the elderly patients with meningiomas.Poor physical functional status,anxiety,fatigue and pain may induce SD.
