摘  要:目的探讨人工智能(AI)系统与内镜医师对胃瘤性病变的诊断效能以及AI系统对于内镜医师的影响。方法收集2021年6月15日至2022年2月9日西安高新医院含有病理活检结果的251例患者的胃部病变图像[图片775张,其中白光443张,内镜窄带成像术(NBI)332张],其中胃瘤性病变64例(图片282张,其中白光168张,NBI 114张)。分析AI系统在白光内镜、NBI结合放大内镜(ME)模式下对于瘤性病变及非瘤性病变的诊断情况;比较AI、内镜医师以及AI辅助内镜医师对于胃瘤性病变的诊断灵敏度、特异度及准确度。结果AI组对于胃瘤性病变的诊断特异度、准确度均高于低年资内镜医师组与AI辅助低年资内镜医师组(P<0.05)。AI组对于胃瘤性病变的诊断特异度低于高年资内镜医师组与AI辅助高年资内镜医师组(P<0.05)。AI辅助低年资内镜医师组与低年资内镜医师组对于胃瘤性病变的诊断灵敏度、特异度、准确度无显著差异(P>0.05)。AI辅助高年资内镜医师组与高年资内镜医师组对于胃瘤性病变的诊断灵敏度、特异度、准确度无显著差异(P>0.05)。结论AI系统的诊断效能优于低年资内镜医师,可在筛查胃瘤性病变过程中指导低年资内镜医师,同时提示低年资内镜医师要不断积累诊断经验,提高胃瘤性病变的诊断灵敏度、特异度、准确度。Objective To explore the diagnostic efficacy of artificial intelligence(AI)system and endoscopists on gastric neoplastic lesions and the influence of AI system on endoscopists.Methods The images of gastric lesions of 251 patients with pathological biopsy results in Xi'an Gaoxin Hospital from June 15,2021 to February 9,2022 were collected[775 pictures,including 443 white light and 332 narrow band imaging(NBI)],including 64 cases of gastric neoplastic lesions(282 pictures,including 168 white light and 114 NBI).The diagnosis of neoplastic and non-neoplastic lesions by AI system in white light endoscopy,NBI combined with magnifying endoscopy(ME)mode were analyzed;the sensitivity,specificity and accuracy of AI,endoscopists and AI-assisted endoscopists in the diagnosis of gastric neoplastic lesions were compared.Results The diagnostic specificity and accuracy of the AI group on gastric neoplastic lesions were higher than those of the junior endoscopists group and the AI-assisted junior endoscopists group(P<0.05).The diagnostic specificity of the AI group on gastric neoplastic lesions was lower than that of the senior endoscopic physician group and the AI-assisted senior endoscopic physician group(P<0.05).There were no significant differences in the diagnostic sensitivity,specificity and accuracy of gastric neoplastic lesions between the AI-assisted junior endoscopy group and the junior endoscopy group(P>0.05).There were no significant differences in the diagnostic sensitivity,specificity and accuracy of the AI-assisted senior endoscopic physician group and the senior endoscopic physician group on gastric neoplastic lesions(P>0.05).Conclusion The diagnostic efficacy of AI system is better than that of junior endoscopists.It can guide junior endoscopists in the screening of gastric neoplastic lesions.At the same time,it is suggested that junior endoscopists should continue to accumulate diagnostic experience and improve the diagnostic sensitivity,specificity and accuracy of gastric neoplastic lesions.
