首发未治疗精神分裂症患者言语性幻听与其自杀观念的相关性分析

摘　　要：目的:探讨首发未治疗精神分裂症(FUSCH)患者言语性幻听(AVHs)与其自杀观念(SI)的关系。方法:收集80例FUSCH患者的一般人口学资料,行精神症状评定量表-幻听分量表(PSYRATS-AH),幻听信念问卷(修订版)(BAVQ-R)、Beck自杀意念量表(BSSI)评估,根据BSSI评估结果将其分为无SI组(n=50)和有SI组(n=30),对两组结果进行统计学分析。结果:有SI组AVHs的PSYRATS-AH总分、BAVQ-R总分、情绪特征、认知解释、权威信念、恶意信念、抵抗幻听得分均高于无SI组患者(P<0.001)。多重线性回归分析显示有SI组AVHs的情绪特征(β=0.258,P=0.020)和权威信念(β=0.197,P=0.036)的分值与其最近1周BSSI分值呈正相关。结论:高水平AVHs的负面情绪和不良信念会增加FUSCH患者的自杀风险。Objective:To explore the relationship between suicidal ideation(SI)in first-episode untreated schizophrenia patients and their Auditory Verbal Hallucinations(AVHs).Method:The general demographic data of 80 first-episode untreated patients with schizophrenia were collected,and the Psychiatric Symptom Assessment Scale-Auditory Hallucinations Project(PSYRATS-AH),the Beliefs About Voices Questionnaire-Revised(BAVQ-R)and Beck Scale for Suicide Ideation(BSSI)were evaluated.According to the evaluation results of BSSI,they were divided into non-SI group(n=50)and SI group(n=30).The results of the two groups were statistically analyzed.Results:PSYRATS-AH total score,BAVQ-R total score,and the scores of emotional characteristics,cognitive interpretation,malevolence,omnipotence and resistance in AVHs in the SI group were higher than those in the non-SI group(P<0.001).Multiple regression analysis showed that the scores of emotional characteristics(β=0.258,P=0.020)and omnipotence(β=0.197,P=0.036)of AVHs in the SI group were positively correlated with their BSSI scores in the last week.Conclusion:High levels of negative emotions and beliefs in AVHs may increase the risk of suicide in the FUSCH patients.
