老年患者内在能力与营养、糖脂代谢的相关性研究

摘　　要：目的探讨老年住院患者内在能力与营养指标及糖脂代谢指标间的关系。方法选取2019年3月至2021年8月于北京医院老年医学科住院的267例≥60岁患者,依据世界卫生组织推荐意见评估老年住院患者的内在能力,并测定血红蛋白、血清白蛋白、前白蛋白、总胆固醇、甘油三酯、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇及血清葡萄糖等生物指标水平。采用独立样本t检验、Spearman相关分析、多元线性回归探讨内在能力与老年患者营养及糖脂代谢指标的关系。结果老年住院患者中95.5%的老年患者存在内在能力减退。内在能力越好,老年患者的血红蛋白、血清白蛋白及前白蛋白水平越高(P<0.05),内在能力的运动功能、活力、认知功能维度水平不同的老年人营养指标具有差异(P<0.05)。调整人口学及疾病因素后内在能力与营养指标仍具有相关性(P<0.05)。内在能力与糖脂代谢指标的关系无统计学意义(P>0.05)。结论老年住院患者的内在能力水平与营养指标具有相关性。Objective To explore the relationship between intrinsic capacity and nutrition,glucose and lipid metabolism indexes in elderly inpatients.Methods A total of 267 inpatients over 60 years old at the Department of Geriatrics in our hospital from March 2019 to August 2021 were included.They were evaluated the intrinsic capacity according to the recommendations of WHO,and the levels of biomarkers such as hemoglobin(Hb),albumin(ALB),prealbumin(PAB),total cholesterol(TC),triglyceride(TG),high density lipoprotein-cholesterol(HDL-C),low density lipoproteincholesterol(LDL-C)and glucose(GLU)were measured.Independent sample t-test,Spearman correlation analysis and multiple linear regression were used to explore the relationship between intrinsic capacity and nutrition and GLU and lipid metabolism indexes in elderly patients.Results 95.5%of elderly patients had decreased intrinsic capacity.The better the intrinsic capacity,the higher the levels of Hb,ALB and PAB in elderly patients(P<0.05).The nutritional indexes of the elderly with different levels of motor function,vitality and cognitive function of intrinsic ability were significantly different(P<0.05).After adjusting for demographic and disease factors,the correlation between intrinsic capacity and nutritional indicators was still significant(P<0.05).The relationship between intrinsic capacity and glycolipid metabolism index did not show difference(P>0.05).Conclusions For elderly inpatients,the intrinsic capacity may be correlated with nutritional indicators.
