摘  要:目的探讨MPR重建和VR重建CT引导在肺寡转移瘤消融术中的应用价值。方法回顾性分析2020年1月至2021年12月在南昌大学第一附属医院高新医院行肺腺癌和结直肠癌肺寡转移瘤消融术患者56例的临床资料,其中31例采用普通轴位CT引导,25例采用MPR重建和VR重建CT引导。分析比较两组完全消融率、并发症发生率、1年局部控制率以及消融病灶的无疾病进展时间的差异。结果常规扫描组和MRP组完全消融率分别为67.7%和92.0%,P=0.028,差异有统计学意义。两组总并发症发生率分别为35.5%和12.0%,P=0.044,差异有统计学意义。1年局部控制率分别为71.0%和92.0%,P=0.049,两组差异有统计学意义。两组的无疾病进展时间分别为14.9个月(95%CI,13.1个月~16.7个月)和17.5个月(95%CI,15.3个月~19.7个月),P=0.210,差异无统计学意义。结论MPR重建和VR重建CT引导较普通轴位CT在肺寡转移瘤消融术中可以提高完全消融率和1年局部控制率,减少并发症,值得临床推广使用。Objective To investigate the application value of MPR reconstruction and VR reconstruction CT guidance in ablation for lung oligometastatic carcinoma.Methods A retrospective analysis was performed on 56 patients with lung adenocarcinoma and colorectal cancer undergoing lung oligo-metastatic tumor ablation in Gaoxin Hospital of the First Affiliated Hospital of Nanchang University from January 2020 to December 2021.Among them,31 patients received conventional axial CT guidance,and 25 patients received MPR reconstruction and VR reconstruction CT guidance.The differences in the rate of complete ablation,complication rate,1-year local control rate,and PFS after ablation were analyzed and compared between the two groups.Results The total ablation rates of the conventional scan group and the MRP group were 67.7%and 92.0%,respectively(P=0.028),with statistical significance.The total complication rates of the two groups were 35.48%and 12%,respectively,P=0.044,and the difference was statistically significant.The 1-year local control rates were 70.96%and 92%,P=0.049,and the difference between the two groups was statistically significant.The mPFS time of the two groups was 14.9 months(95%CI,13.1-16.7 months)and 17.5 months(95%CI,15.3-19.7 months),respectively,P=0.210,and the difference was not statistically significant.Conclusions Compared with conventional axial CT,MPR reconstruction and VR reconstruction CT guidance can improve the complete ablation rate and one-year local control rate in the ablation of lung oligometastatic tumors,and reduce complications,which is worthy of clinical application.
