涤痰汤联合丙戊酸钠治疗痰浊阻窍型强直-阵挛发作癫痫患儿的效果

摘　　要：目的:观察涤痰汤联合丙戊酸钠治疗痰浊阻窍型强直-阵挛发作癫痫患儿的效果。方法:选取2017年12月至2019年12月该院收治的90例痰浊阻窍型强直-阵挛发作癫痫患儿进行前瞻性研究,按照抽签法将其分为对照组和观察组各45例。两组均进行基础治疗,在此基础上,对照组予以丙戊酸钠治疗,观察组在对照组基础上联合涤痰汤治疗,两组均连续治疗3个月。比较两组临床疗效、治疗前后中医证候积分、炎性因子[超敏C反应蛋白(hs-CRP)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)]水平和不良反应发生率。结果:观察组治疗总有效率为95.56%(43/45),高于对照组的77.78%(35/45),差异有统计学意义(P<0.05);观察组主症、次症、舌脉等中医证候积分和hs-CRP、IL-6、TNF-α水平均低于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:涤痰汤联合丙戊酸钠治疗痰浊阻窍型强直-阵挛发作癫痫患儿可提高治疗总有效率,降低中医证候积分和炎性因子水平,效果优于单纯丙戊酸钠治疗。Objective:To observe effects of Ditan decoction combined with Sodium valproate in treatment of children with tonic-clonic seizure epilepsy of turbid phlegm blocking the clear orifices type.Methods:A prospective study was conducted on 90 children with tonicclonic seizure epilepsy of turbid phlegm blocking the clear orifices type admitted to the hospital from December 2017 to December 2019.They were divided into control group and observation group according to the lottery method,45 cases in each.Both groups received basic treatment.On this basis,the control group was treated with Sodium valproate,while the observation group was treated with Ditan decoction on the basis of that of the control group.Both groups were treated for 3 months.The clinical efficacy,the TCM syndrome scores before and after the treatment,the levels of inflammatory factors[high-sensitivity C-reactive protein(hs-CRP),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)],and the incidence of adverse reactions were compared between the two groups.Results:The total effective rate of treatment in the observation group was 95.56%(43/45),which was higher than 77.78%(35/45)in the control group,and the difference was statistically significant(P<0.05).The scores of TCM syndromes such as main symptoms,secondary symptoms,tongue and pulse and the levels of hs-CRP,IL-6 and TNF-αin the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions:Ditan decoction combined with Sodium valproate can improve the total effective rate of treatment and reduce the scores of TCM syndromes and the levels of inflammatory factors in the treatment of the children with tonic-clonic seizure epilepsy of turbid phlegm blocking the clear orifices type.Moreover,it is superior to single Sodium valproate treatment.
