摘  要:目的:探讨神经肌肉电刺激联合吞咽功能训练治疗卒中后吞咽障碍患者的效果。方法:选择2019年4月至2021年11月该院收治的85例卒中后吞咽障碍患者进行前瞻性研究,按随机数字表法分为对照组(n=42)与观察组(n=43)。对照组采用吞咽功能训练治疗,观察组在对照组基础上加用神经肌肉电刺激治疗,两组均治疗8周。比较两组治疗前后洼田饮水试验分级、吞咽障碍特异性生活质量量表(SWAL-QOL)评分及并发症发生率。结果:治疗后,观察组洼田饮水试验分级优于对照组,进食时间、食欲、食物选择、症状频率等SWAL-QOL维度评分均高于对照组,差异有统计学意义(P<0.05);观察组并发症发生率为4.65%(2/43),低于对照组的21.43%(9/42),差异有统计学意义(P<0.05)。结论:神经肌肉电刺激联合吞咽功能训练治疗卒中后吞咽障碍患者效果优良,能有效改善洼田饮水试验分级,降低并发症发生率,提高患者生命质量,效果优于单用吞咽功能训练。Objective:To explore effects of neuromuscular electrical stimulation combined with swallowing function training on patients with dysphagia after stroke.Methods:A prospective study was conducted on 85 patients with dysphagia after stroke admitted to the hospital from April 2019 to November 2021.According to the random number table method,they were divided into control group(n=42)and observation group(n=43).The control group was treated with swallowing function training,while the observation group was treated with neuromuscular electrical stimulation on the basis of that of the control group.Both groups were intervened for 8 weeks.The Kubota water swallow test grading,the swallowing quality of life scale(SWAL-QOL)score,and the incidence of complications were compared between the two groups before and after the treatment.Results:After the treatment,the grading of Kubota water swallow test in the observation group was better than that in the control group,d the SWAL-QOL scores of feeding time,appetite,food selection and symptom frequency were higher than those in the control group,and the differences were statistically significant(P<0.05).The incidence of complications in the observation group was 4.65%(2/43),which was lower than 21.43%(9/42)in the control group,and the difference was statistically significant(P<0.05).Conclusions:Neuromuscular electrical stimulation combined with swallowing function training is effective in the treatment of the patients with dysphagia after stroke.It can effectively improve the grading of Kubota water swallow test,reduce the incidence of complications and improve the quality of life.Moreover,it is superior to single swallowing function training.
