摘 要:目的:分析第二磨牙全冠修复后食物嵌塞的临床特征,并对咬合接触情况进行量化研究。方法:纳入完成第二磨牙(M2)全冠修复的1152例的患者进行随访,计算食物嵌塞的发生率。对其中110例随访资料完整的患者嵌塞类型、嵌塞出现时间以及食物嵌塞原因等进行分析,同时采用数字化技术对邻接未丧失的全冠及其邻牙的咬合接触情况进行量化分析。结果:1152例M2全冠修复后患者351例(30.5%)发生M2全冠与邻牙食物嵌塞,且发生率随修复年限增加而增高。在纳入研究的110例食物嵌塞患者中85例为M2与第一磨牙(M1)间嵌塞,其中63例(74.1%)为邻接紧密型食物嵌塞。嵌塞的全冠近中斜面/远中斜面咬合接触面积比显著大于健康对侧同名牙(P<0.01)。结论:M2全冠修复后食物嵌塞发生率高,邻接紧密型占比较大,咬合接触不平衡是导致其发生的潜在原因。Objective:To analyze the clinical characteristics of food impaction of the second molars(M2)after crown restoration and quantitatively analyze the occlusal contacts.Methods:1152 patients underwent crown restoration of the M2 were included for the study.The type of impaction,the time of impaction and the causes of food impaction were analyzed in 110 patients with completed follow-up data after crown restoration.At the same time,digital technology was used to quantify the occlusal contact of the crown with the tight proximal contact of the adjacent teeth.Results:Food impaction was found in 351(30.5%)of the 1152 patients after crown restoration for the M2 and the incidence increased with the time after restoration.85 cases with food impaction between M2 and the first molar(M1),63 of them(74.1%)were with tight proximal contact.The ratio of occlusal contact area between mesial surface and distal surface of the cusps of M2 with food impaction was significantly greater than that of the healthy control(P<0.01).Conclusion:The incidence of food impaction after crown restoration of the M2 is high,and the proportion of food impaction with the tight proximal contacts is large.Unbalanced occlusal contact is the underlying cause of its occurrence.
