摘 要:目的:用三维有限元分析方法研究替牙期单侧完全性唇腭裂(UCLP)患者上颌骨在静息状态口周力作用下的生物力学特点。方法:提取1名替牙期UCLP患者的CBCT图像,建立上颌骨复合体三维有限元模型,选择双侧切牙区、尖牙区、第一和第二前磨牙/乳磨牙区、第一恒磨牙区的唇(颊)舌侧的牙槽突区作为施力部位,垂直于牙槽突表面施加前期研究所获得的口周力值,分析模拟口周力作用后上颌骨的应力及位移变化。结果:在口周力的作用下,上颌骨的健、裂侧在三维方向上均发生应力及位移的改变。VonMises应力分布比较均匀。位移的方向主要是向后向内,前段向后移位最明显,健侧中后段大于裂侧。健、裂侧和前后部的抑制作用分别呈现出不对称性及不均衡性,唇颊侧压力较大或(和)舌侧压力较小。结论:口周力对UCLP患者上颌骨的生长发育在三维方向上有不同程度地抑制,矢状向抑制程度最大,前段受抑制最明显。Objective:To study the biomechanical characteristics of perioral force on the maxilla in patients with unilateral complete cleft lip and palate(UCLP)after surgery in the mixed dentition by 3D finite element analysis method.Methods:The CBCT image of a patient with mixed dentition and UCLP was extracted,and a 3D finite element model of the maxillary complex was established.The alveolar process areas of the lip(buccal)of the bilateral incisors,canines,first and second premolars/deciduous molars,and the first molars were selected as the force application sites,and the perioral force value obtained in the previous study was applied perpendicularly to the surface of the alveolar process,and the stress and displacement of the maxilla after the application of simulated perioral force were observed and analyzed.Results:Under the action of the perioral force,the healthy and cleft side of the maxilla underwent changes in 3D stress and displacement.The Von Mises stress distribution was relatively uniform.The direction of displacement was mainly backward and inward,and the backward displacement of the anterior segment was the most obvious.The middle and posterior segment of the healthy side was larger than that of the cleft side.the pressure on the buccal side was too large or(and)that on the lingual side was too small.Conclusion:Perioral force may inhibit the growth and development of maxilla in UCLP patients in the 3D direction.The inhibitory effect in the sagital derection is the most and more obvious on the anterior segment.
