摘  要:目的探讨依达拉奉右莰醇联合血管内介入治疗对大面积脑梗死(MCI)患者血清NSE、S100β、ROS、NO、MDA及GSH-px水平的影响。方法选取2020年1月至2021年12月海南医学院第一附属医院神经内科收治的110例MCI患者,随机分为观察组(55例)和对照组(55例)。对照组采用血管内介入治疗,观察组在此基础上加用依达拉奉右莰醇。对比两组临床疗效、不良反应差异,观察治疗前后两组患者NIHSS评分、血清神经元特异性烯醇化酶(NSE)、S100β、活性氧簇(ROS)、一氧化氮(NO)、丙二醛(MDA)及谷胱甘肽过氧化物酶(GSH-px)变化。结果观察组治疗总有效率显著高于对照组(P<0.05)。治疗3d及14d时,观察组患者NIHSS评分、血清ROS、NO、MDA水平及血清NSE、S100β水平低于对照组,血清GSH-px水平显著高于对照组(P<0.05)。两组治疗不良反应发生率差异无统计学意义(P>0.05)。结论依达拉奉右莰醇联合血管内介入治疗MCI的疗效显著,能有效降低患者氧化应激水平,改善神经功能,且安全性高。Objective To investigate the effects of edaravone dexcanol combined with intravascular interventional therapy on serum levels of NSE,S100β,ROS,NO,MDA and GSH-px in patients with massive cerebral infarction(MCI).Methods A total of 110 patients with MCI admitted to our hospital from January 2020 to December 2021 were randomly divided into observation group(55 cases)and control group(55 cases).The control group was treated with endovascular interventional therapy,and the observation group was treated with edaravone dexcanol on this basis.The clinical efficacy and adverse reactions of the two groups were compared.The changes of NIHSS score,serum neuron specific enolase(NSE),S100β,reactive oxygen species(ROS),nitric oxide(NO),malondialdehyde(MDA)and glutathione peroxidase(GSH-Px)were observed before and after treatment in 2 groups.Results The total effective rate in observation group was significantly higher than that in control group(P<0.05).After 3 days and 14 days of treatment,the NIHSS score,serum ROS,NO,MDA levels and serum NSE and S100βlevels in observation group were lower than those in control group,and serum GSH-Px level was significantly higher than that in control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Edaravone dextran combined with endovascular intervention is effective in the treatment of MCI,which can effectively reduce the level of oxidative stress and improve the neurological function,with high safety.
