基于聚类分析和关联规则探讨脑小血管病相关认知功能障碍的中医证候规律

摘　　要：目的:基于聚类分析和关联规则探讨脑小血管病相关认知功能障碍的中医证候规律。方法:采用横断面研究,收集102例脑小血管病相关认知功能障碍病人的中医症状、舌象、脉象等四诊信息,对四诊信息进行聚类分析、关联规则Apriori算法统计分析。结果:聚类分析显示,脑小血管病相关认知功能障碍可聚为5类,类一为心悸,神疲,气短,乏力,视物模糊,目涩,耳鸣,失眠,多梦,急躁易怒,善太息,腹胀;类二为头晕,头痛,胸闷,五心烦热,潮热盗汗,舌暗,苔白腻;类三为头重如裹,面色萎黄,齿摇,口黏腻,肢倦身重,畏寒肢冷,肢体疼痛,麻木,腰膝酸软,小便清长,咳笑时遗尿,夜尿频多,尿后余沥;类四为肢体笨拙,面色晦暗,口唇紫暗,面红,自汗,尿急,小便短赤,舌下瘀点瘀斑,脉弦;类五为咳痰,口渴,口苦,形体肥胖,舌红,苔黄。Apriori算法显示,该疾病核心症状组合为头晕-腰膝酸软-视物模糊-目涩;夜尿频多-腰膝酸软-视物模糊-目涩;口苦-腰膝酸软-视物模糊-目涩;头晕-腰膝酸软-视物模糊;夜尿频多-腰膝酸软-视物模糊;口苦-腰膝酸软-视物模糊。结论:脑小血管病相关认知功能障碍的中医证候要素虚实并见,虚证以阳虚、阴虚、气虚为主,实证以火热、痰浊、血瘀、气滞为主,病位涉及肝、脾、肾、胃,常见的中医证型为肝郁脾虚证、肝肾阴虚证、脾肾阳虚证、瘀热阻窍证、痰热阻窍证,其核心中医证候为肝肾阴虚证。Objective:To analyze the regularity of traditional Chinese medicine symptom of cognitive dysfunction with cerebral small vessel disease based on cluster analysis and association rule.Methods:A cross-sectional study was used to collect four diagnostic information such as traditional Chinese medicine symptoms,tongue signs and pulse signs from 102 patients of cognitive dysfunction with cerebral small vessel disease,and the four diagnostic information was analyzed by cluster analysis and association rule Apriori algorithm.Results:Cluster analysis showed that cognitive dysfunction with cerebral small vessel disease could be clustered into five categories.Category 1:palpitation,exhaustion,shortness of breath,fatigue,blurred vision,dry eye,tinnitus,insomnia,dreaminess,irritability,heave a sigh,ventosity;category 2:dizziness,headache,chest tightness,dysphoria in chest palms-soles,hot flushes,night sweats,dark tongue,white greasy moss;category 3:head heavy as wrapped,pale yellow,teeth shaking,sticky mouth,body weight,fear of cold limb cold,limb pain,numbness,waist and knee pain,urine clear,cough and laughter enuresis,frequent night urination,residual draining after urine;category 4:clumsy limbs,dark complexion,dark purple lips,red face,spontaneous sweating,urgent urination,short red urination,petechial spots under the tongue,pulse string;category 5:phlegm,thirst,bitter mouth,body fat,red tongue,yellow moss.Apriori algorithm showed that the combination of core symptoms of the disease was dizziness-waist and knee pain-blurred vision-astringent vision;frequent night urination-waist and knee pain-blurred vision-astringent vision;bitter mouth-sore waist and knee-blurred vision-astringent vision;dizziness-sore waist and knee-blurred vision;frequent night urination-waist and knee pain-blurred vision;bitter mouth-sore waist and knees-blurred vision.Conclusion:The traditional Chinese medicine syndrome elements of cognitive impairment with cerebral small vessel disease were both deficiency,and excess.The deficiency syndrome was Yan
