腹腔热灌注化疗联合肿瘤细胞减灭术对卵巢癌患者的治疗效果

摘　　要：目的 探讨腹腔热灌注化疗联合肿瘤细胞减灭术对卵巢癌患者的治疗效果.方法 根据治疗方法的不同将135例卵巢癌患者分为对照组(n=67)和观察组(n=68),两组患者均予以肿瘤细胞减灭术,对照组患者联合常规腹腔灌注化疗,观察组患者联合腹腔热灌注化疗,两组患者均予以认知-存在团体干预.比较两组患者的临床疗效、术中和术后指标、血管内皮生长因子(VEGF)水平及免疫功能.结果 观察组患者的总有效率高于对照组,术后住院时间短于对照组,1年复发率和病死率均低于对照组,差异均有统计学意义(P﹤0.05).治疗后1个月,两组患者的血清VEGFA、VEGFB、VEGFC水平均低于本组治疗前,CD3^(+)、CD4^(+)水平及CD4^(+)/CD8^(+)均高于本组治疗前,观察组患者的血清VEGFA、VEGFB、VEGFC水平均低于对照组,CD3^(+)、CD4^(+)水平及CD4^(+)/CD8^(+)均高于对照组,差异均有统计学意义(P﹤0.05).结论 腹腔热灌注化疗联合肿瘤细胞减灭术可提高卵巢癌患者的临床疗效,缩短住院时间,降低血清肿瘤标志物水平、1年复发率及病死率,改善免疫功能.Objective To investigate the therapeutic effect of intraperitoneal hyperthermic perfusion chemotherapy combined with tumor cytoreductive surgery on patients with ovarian cancer.Method A total of 135 ovarian cancer patients were divided into control group(n=67)and observation group(n=68)according to different treatment methods.Both groups received tumor cytoreductive surgery,patients in the control group received the regimen combined with conventional intraperitoneal chemotherapy,and patients in the observation group received the regimen combined with intraperitoneal hyperthermic perfusion chemotherapy,both groups received cognitive-existence group intervention.The clinical curative effect,intraoperative and postoperative indicators,vascular endothelial growth factor(VEGF)level,and immune function of the two groups were compared.Result The total effective rate of the observation group was higher than that of the control group,the postoperative hospital stay was shorter than that of the control group,and the 1-year recurrence rate and mortality were lower than those of the control group,and the differences were statistically significant(P<0.05).One month after the treatment,the levels of serum VEGFA,VEGFB,and VEGFC in the two groups were lower than those before the treatment,and the levels of CD3^(+),CD4^(+),and CD4^(+)/CD8^(+)were higher than those before the treatment,the levels of serum VEGFA,VEGFB,and VEGFC in the observation group were lower than those in the control group,and the levels of CD3^(+),CD4^(+),and CD4^(+)/CD8^(+)in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).Conclusion Intraperitoneal hyperthermic perfusion chemotherapy combined with tumor cytoreductive surgery would improve the clinical efficacy of ovarian cancer patients,shorten the length of hospital stay,reduce the level of serum tumor markers,1-year recurrence rate and mortality,and improve their immune function.
