鼻内窥镜下鼻腔泪囊造孔术联合贝复舒及典必殊治疗慢性泪囊炎的效果分析

摘　　要：目的分析鼻内窥镜下鼻腔泪囊造孔术联合重组牛碱性成纤维细胞生长因子眼用凝胶(商品名:贝复舒)及妥布霉素地塞米松滴眼液(商品名:典必殊)治疗慢性泪囊炎(CD)的临床效果。方法80例CD患者为研究对象,依据随机数字表法分为研究组与对照组,各40例。两组均行鼻内窥镜下鼻腔泪囊造孔术,对照组患者以支撑管为支撑物,研究组患者则以贝复舒及典必殊为支撑。对比两组治疗前后鼻腔分泌物炎性因子[白细胞介素-8(IL-8)、白细胞介素-6(IL-6)及肿瘤坏死因子-α(TNF-α)]水平、眼表症状评分、生活质量评分及治疗3个月后溢泪程度评分和泪膜破裂时间。结果治疗3个月后,研究组TNF-α、IL-8、IL-6水平分别为(26.35±3.27)、(83.79±9.65)、(77.48±9.91)pg/ml,均低于对照组的(32.53±3.86)、(103.63±11.25)、(100.91±12.11)pg/ml,差异均有统计学意义(P<0.05)。治疗3个月后,研究组眼表症状评分(3.65±1.23)分低于对照组的(5.93±1.36)分,差异有统计学意义(P<0.05)。治疗3个月后,研究组溢泪程度评分(0.95±0.03)分显著低于对照组的(1.10±0.09)分,泪膜破裂时间(12.97±1.61)s长于对照组的(11.22±1.55)s,差异有统计学意义(P<0.05)。治疗3个月后,研究组生活质量评分(90.50±7.52)分高于对照组的(85.28±8.35)分,差异有统计学意义(P<0.05)。结论临床采用鼻内窥镜下鼻腔泪囊造孔术联合贝复舒及典必殊治疗CD患者,可改善患者视力,减轻鼻腔分泌物炎性因子,能提高其生活质量。Objective To analyze the clinical effect of endoscopic endonasal dacryocystorhinostomy combined with recombinant bovine basic fibroblast growth factor eye-gle(trade name:Beifushu)and Tobradex for the treatment of chronic dacryocystitis(CD).Methods A total of 80 patients with CD were studied,and were divided into research group and control group according to the random numerical table,with 40 cases in each group.Both groups underwent endoscopic endonasal dacryocystorhinostomy.Patients in the control group were supported by a support tube,while patients in the research group were supported by Tobradex and Beifushu.Both groups were compared in terms of levels of inflammatory factors[interleukin-8(IL-8),interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)]in nasal secretions before and after treatment,ocular surface symptom score,quality of life score,epiphora score and tear film break-up time 3 months after treatment.Results 3 months after treatment,the levels of TNF-α,IL-8 and IL-6 were(26.35±3.27)pg/ml,(83.79±9.65)pg/ml,and(77.48±9.91)pg/ml in the research group,which were lower than those of(32.53±3.86),(103.63±11.25)and(100.91±12.11)pg/ml in the control group,and the differences were statistically significant(P<0.05).3 months after treatment,the ocular surface symptom score of(3.65±1.23)points in the research group was lower than that of(5.93±1.36)points in the control group,and the difference was statistically significant(P<0.05).3 months after treatment,the epiphora score of(0.95±0.03)points in the research group was significantly lower than that of(1.10±0.09)points in the control group;the tear film break-up time of(12.97±1.61)s in the research group was longer than that of(11.22±1.55)s in the control group;the differences were statistically significant(P<0.05).3 months after treatment,the quality of life score of(90.50±7.52)points in the research group was higher than that of(85.28±8.35)points in the control group,and the difference was statistically significant(P<0.05).Conclusion Endoscopi
