血浆载脂蛋白J对阿尔茨海默病的诊断价值

摘　　要：目的 探讨血浆载脂蛋白J(apolipoprotein J,Apo J)对阿尔茨海默病(Alzheimer’s disease,AD)的诊断价值。方法 选取首都医科大学宣武医院2019年1月1日至2022年12月30日收治的68例AD患者为观察组,另选择首都医科大学宣武医院同期招募的66例健康受试者为对照组。比较分析两组受试者的脑脊液相关指标、简易智力状态检查量表(mini-mental state examination,MMSE)评分以及血浆ApoJ水平。分析血浆ApoJ水平与MMSE评分的相关性以及血浆ApoJ对AD的诊断价值。结果 观察组的MMSE评分、脑脊液Aβ42水平、血浆ApoJ水平均显著低于对照组(P<0.05),脑脊液P-tau、脑脊液T-tau水平均显著高于对照组(P<0.05)。Spearman相关性分析结果显示,血浆ApoJ水平与MMSE评分呈显著正相关(r=0.514,P<0.001)。受试者工作特征曲线分析结果显示,血浆ApoJ诊断AD的曲线下面积为0.825,敏感度为0.853,特异度为0.652。结论 AD患者的血浆ApoJ水平明显降低,且血浆ApoJ对AD具有一定的诊断价值。Objective To investigate the diagnostic value of plasma apolipoprotein J(ApoJ)for Alzheimer’s disease(AD).Method 68 AD patients admitted to Xuanwu Hospital,Capital Medical University from January 1,2019 to December 30,2022 were selected as the observation group.66 healthy subjects recruited from Xuanwu Hospital,Capital Medical University during the same period were selected as the control group.Cerebrospinal fluid related indexes,mini-mental state examination(MMSE)scores and plasma ApoJ levels in the two groups were compared and analyzed.The correlation between plasma ApoJ level and MMSE score and the diagnostic value of plasma ApoJ level for AD were analyzed.Result The MMSE score,cerebrospinal fluid Aβ42 level and plasma ApoJ level in observation group were significantly lower than those in control group(P<0.05),and the levels of P-tau and T-tau in cerebrospinal fluid were significantly higher than those in control group(P<0.05).Spearman correlation analysis showed that plasma ApoJ level was significantly positively correlated with MMSE score(r=0.514,P<0.001).The analysis results of ROC curve showed that the area under the curve,sensitivity and specificity of plasma ApoJ in the diagnosis of AD were 0.825,0.853 and 0.652.Conclusion The plasma ApoJ level in AD patients is significantly decreased,and the plasma ApoJ has certain diagnostic value for AD.
