摘　　要：目的 探讨P63、CK20在非肌层浸润性膀胱癌(non-muscle invasive bladder cancer,NMIBC)中的表达及其与NMIBC复发的相关性。方法 回顾性分析2016年1月至2021年10月河北省承德市中心医院收治的79例做经尿道膀胱肿瘤电切术的NMIBC患者的临床资料。比较分析不同肿瘤分期和分级患者的P63、CK20阳性表达率。分析P63、CK20阳性表达率与肿瘤分期和分级的相关性以及P63与CK20的相关性。根据患者的P63阳性表达情况将其分为P63阳性组(56例)和P63阴性组(23例);根据患者的CK20阳性表达情况将其分为CK20阳性组(65例)和CK20阴性组(14例)。分析P63、CK20阳性表达情况与NMIBC复发的相关性。结果 T1期患者的P63阳性表达率和CK20阳性表达率均显著高于Ta期患者(P<0.05)。高危患者的P63阳性表达率和CK20阳性表达率均显著高于低危患者(P<0.05)。Spearman相关性分析结果显示,P63的阳性表达率与肿瘤分期、分级均呈显著正相关(r=12.453,P<0.05;r=8.103,P<0.05);CK20的阳性表达率与肿瘤分期、分级均呈显著正相关(r=4.355,P<0.05;r=15.964,P<0.05);P63与CK20呈显著正相关(r=6.395,P<0.05)。Kaplan-Meier生存曲线分析结果显示,P63阳性组患者随访期间的无复发生存率显著低于P63阴性组(P<0.001);CK20阳性组患者随访期间的无复发生存率显著低于CK20阴性组(P<0.001)。结论 P63、CK20的阳性表达与NMIBC的分期、分级和复发均显著相关。Objective To investigate the expression of P63 and CK20 in non-muscle invasive bladder cancer(NMIBC)and their correlation with the recurrence of NMIBC.Method The clinical data of 79 patients with NMIBC who underwent transurethral resection for bladder tumor admitted to our hospital from January 2016 to October 2021 were retrospectively analyzed.The positive expression rates of P63 and CK20 in patients with different tumor stages and grades were compared and analyzed.The correlation between the positive expression rate of P63,CK20 and tumor stage,tumor grade,the correlation between P63 and CK20 were analyzed.Patients were divided into P63 positive group(56 cases)and P63 negative group(23 cases)according to the positive expression of P63.Patients were divided into CK20 positive group(65 cases)and CK20 negative group(14 cases)according to the positive expression of CK20.The correlation between the positive expression of P63 and CK20 and the recurrence of NMIBC was analyzed.Result The positive expression rates of P63 and CK20 in T1 patients were significantly higher than those in Ta patients(P<0.05).The positive expression rates of P63 and CK20 in high-risk patients were significantly higher than those in low-risk patients(P<0.05).Spearman correlation analysis showed that the positive expression rate of P63 was significantly positively correlated with tumor stage and grade(r=12.453,P<0.05;r=8.103,P<0.05);the positive expression rate of CK20 was significantly positively correlated with tumor stage and grade(r=4.355,P<0.05;r=15.964,P<0.05);P63 was positively correlated with CK20(r=6.395,P<0.05).Kaplan-Meier survival curve analysis showed that the relapse-free survival rate in P63 positive group was significantly lower than that in P63 negative group(P<0.001),the relapse-free survival rate in CK20 positive group was significantly lower than that in CK20 negative group(P<0.001).Conclusion The positive expression of P63 and CK20 was significantly correlated with the staging,grading and recurrence of NMIBC.
