舌侧矫治器对成人睡眠呼吸影响的相关性研究

摘  要:目的验证舌侧矫治器对睡眠呼吸、上气道及舌骨位置无影响的假设,并探讨舌侧矫治器对睡眠呼吸的影响。方法本研究为病例对照研究,纳入31例骨性Ⅰ类错畸形的成年患者,分为唇侧矫治器组及舌侧矫治器组。在矫治前和矫治开始约5个月时利用睡眠监测仪获取呼吸暂停低通气指数(apneahypopneaindex,AHI)并拍摄锥形束CT分析上气道体积、最小横截面积及舌骨位置的变化。结果舌侧组治疗前腭咽最小横截面积为(247.00±112.21) mm2,治疗后为(209.33±93.93) mm2,缩小(37.67±65.52) mm2 (P=0.026);舌咽最小横截面积治疗前为(252.61±101.17) mm2,治疗后为(204.56±61.19) mm2,缩小(48.06±58.07) mm2 (P=0.03);腭咽体积治疗前为(10.74±4.03) cm3,治疗后为(9.87±3.45) cm3,缩小(0.87±1.71) cm3 (P=0.046)。结论舌侧矫治器组腭咽段及舌咽段发生缩窄,但AHI值稳定,因此舌侧矫治器会影响气道,但对睡眠呼吸无影响。Objective To test the hypothesis that the lingual appliance has no effect on breathing during sleep,pharyngeal airway dimension and hyoid bone position and to investigate the influence of the lingual appliance on breathing during sleep.Methods This study is a case-control study.Thirty-one adults with ClassⅠmalocclusion were divided into lingual appliance group and labial appliance group.Apnea Hypopnea Index(AHI)was monitored by Polysomnography(PSG)during sleep before treatment and after almost 5 months of treatment,and Cone-beam CT was performed to analyze the changes in pharyngeal airway volume,minimum axial area and hyoid bone position.Results In the lingual appliance group,the minimum axial area of the velopharyngeal area was(247.00±112.21)mm2 before treatment and 209.33±93.93 mm2 after treatment,with a reduction of(37.67±65.52)mm2(P=0.026).The minimum axial area of the glossopharyngeal area was(252.61±101.17)mm2 before treatment and(204.56±61.19)mm2 after treatment,with a reduction of(48.06±58.07)mm2(P=0.03).The velopharyngeal volume was(10.74±4.03)cm3 before treatment and(9.87±3.45)cm3 after treatment,with a reduction of(0.87±1.71)cm3(P=0.046).Conclusions In the lingual appliance group,while the dimension of the velopharyngeal and glossopharyngeal areas was constricted,the value of AHI was stable.The lingual appliance might affect the pharyngeal airway dimension,but does not cause sleep apnea.
