明目地黄丸联合人工泪液滴眼治疗白内障术后干眼症的疗效研究

摘　　要：目的探讨明目地黄丸联合人工泪液滴眼治疗白内障术后干眼症的疗效。方法选择2019年6月-2022年6月古田县医院收治的有白内障并行超声乳化术后发生干眼症的患者90例,按照随机数字表法分为对照组和研究组各45例。对照组患者术后应用聚乙烯醇滴眼液进行治疗,观察组在聚乙烯醇滴眼液基础上口服明目地黄丸进行治疗,连续治疗7 d。比较两组患者临床疗效、泪膜破裂时间(tear film break-up time,BUT)、SchirmerⅠ试验结果(Schirmer Ⅰ test results,SITI)、角膜荧光素染色水平(corneal fluorescein staining level,FL)及国际标准眼表疾病指数干眼自查问卷(international standard ocular surface disease index,OSDI)评分。结果观察组治疗总有效率95.56%高于对照组的80.00%(P<0.05);治疗后观察组BUT、SITI均高于对照组(均P<0.05),FL低于对照组(P<0.05);治疗后两组患者OSDI评分均降低(P<0.05),且观察组治疗后OSDI评分低于对照组(P<0.05)。结论明目地黄丸联合人工泪液滴眼治疗白内障术后干眼症疗效确切,可延长泪膜破裂时间,增加泪液分泌,促进角膜上皮修复,改善视功能。Objective To investigate the efficacy of Mingmu Dihuang Wan combined with artificial tear drops in the treatment of dry eye after cataract surgery.Methods A total of 90 patients admitted to Gutian County Hospital from June 2019 to June 2022 who had dry eye syndrome after cataract phacoemulsification were selected and divided into the control group(N=45 cases)and the study group(N=45 cases)according to the random number table method.The patients in the control group were treated with polyvinyl alcohol drops after surgery,and the observation group was treated with Mingmu Dihuangwan orally on the base of polyvinyl alcohol drops for 7 d.The clinical efficacy,tear film break-up time(BUT),Schirmer Ⅰ test results(SITI),corneal fluorescein staining level(FL)and International standard ocular surface disease index(OSDI)of dry eye self-examination questionnaire scores were compared between two groups.Results The total effective rate in the observation group was 95.56%,which was significantly higher than 80.00%in the control group(P<0.05).BUT and SITI in the observation group were significantly higher than those in the control group after treatment(all P<0.05),and FL was significantly lower than that in the control group(P<0.05).OSDI scores were significantly lower in both groups after treatment(P<0.05),and OSDI scores in the observation group were significantly lower than those in the control group after treatment(P<0.05).Conclusion The efficacy of Mingmu Dihuangwan combined with artificial tear drops in the treatment of dry eye after cataract surgery is definite,which can prolong tear film rupture time,increase tear secretion,promote corneal epithelial repair,and improve visual function.
