基于数据挖掘的中药汤剂治疗围绝经期焦虑抑郁的用药规律分析及量效关系

摘　　要：目的:基于数据挖掘探究中药汤剂治疗围绝经期焦虑抑郁的用药规律、核心组方及其量效关系。方法:检索中国学术期刊网络出版总库(CNKI)、中国医学生物文献数据库(CBM)、万方数据库及维普中文科技期刊全文数据库(VIP)收录的中药治疗围绝经期焦虑抑郁的临床研究文献进行分析,总结用药规律和核心组方。结果:纳入处方27首,涉及中药105味。用药频次较高的有甘草、柴胡、茯苓、白芍等中药37味,功效多为补虚药;药性统计气味、归经以寒性、温性、甘味、心经、肝经最多。关联规则分析得到以甘草、柴胡、茯苓、白芍等为核心的组方。量效分析显示柴胡、甘草、牡蛎、淫羊藿4味药作为主药的剂量与非主药相比差异有统计学意义(P<0.01)。相关性分析显示,白芍与当归,生地黄与甘草,龙骨与柴胡等的剂量之间分别有统计学意义(P<0.05)。结论:围绝经期焦虑抑郁主要治法为疏肝解郁、养心安神。治疗首选柴胡、茯苓、白芍、当归等药物。Objective:To explore the medication rule,core prescription and dose-effect relationship of TCM medicine decoction in the treatment of perimenopausal anxiety and depression based on data mining.Methods:The clinical research literature of TCM medicine in the treatment of perimenopausal anxiety and depression collected in CNKI,CBM,Wanfang Database and VIP Database were analyzed to summarize the medication rules and core prescriptions.Results:A total of 27 prescriptions were included,involving 105 kinds of TCM medicine.The frequently used TCM medicines include 37 TCM medicines,such as Gancao(Radix Glycyrrhizae),Chaihu(Radix Bupleuri),Fuling(Poria)and Baishao(Radix Paeoniae Alba),most of them are TCM medicine for tonifying deficiency.The four Qi(气)and five flavors and channel tropism of TCM medicines are counted,among which cold,warm,sweet,heart meridian and liver meridian are more.The result of association rule analysis shows that prescription,with Gancao,Chaihu,Fuling and Baishao as the core medicines.The results of dose-effect analysis showed that the dose of Chaihu,Gancao,Muli(Concha Ostreae)and Yinyanghuo(Herbaa Epimedii)as the main medicines were significantly different from that when not as the main medicines(P<0.01).Correlation analysis showed that there were significant differences between the doses of Baishao and Danggui(Radix Angelicae Sinensis),Shengdihuang(Radix Rehmanniae)and Gancao,Longgu(Os Draconis)and Chaihu(P<0.05).Conclusion:The main treatment of perimenopausal anxiety and depression is soothing liver and relieving depression,nourishing heart and tranquilizing mind.Chaihu,Fuling,Baishao,Danggui are the first choice for treatment.
