摘　　要：目的探讨let-7a在喉癌组织中的表达及与其临床病理特征的关系。方法将80例经病理确诊为喉癌的患者作为研究对象。采用实时荧光定量聚合酶链反应(qRT-PCR)检测let-7a水平。分析let-7a水平与喉癌临床病理特征的关系,受试者特征曲线(ROC)分析血清let-7a水平对喉癌的诊断效能。结果喉癌患者let-7a表达量在不同分化程度、TNM分期、淋巴结转移方面比较,P<0.05,在性别、年龄、肿瘤类型、吸烟史及饮酒史方面比较,P>0.05。ROC曲线分析显示,曲线下面积(AUC)为0.851,95%置信区间为0.775~0.933,灵敏度为73.9%,特异度为78.2%,let-7a相对表达量的最佳临界值为0.77。结论let-7a水平在喉癌患者中呈现异常低表达的现象,其水平变化与患者分化程度、TNM分期、淋巴结转移有关,且可作为预测喉癌发病风险敏感指标。Objective To investigate the expression of let-7a in laryngeal carcinoma and its relationship with clinicopathologic features.Methods 80 cases of laryngeal cancer diagnosed by pathology were studied.Real-time quantitative fluorescence polymerase chain reaction(qRT-PCR)was used to detect let-7a levels.The relationship between let-7a level and clinicopathologic features of laryngeal cancer was analyzed,and the diagnostic efficacy of serum let-7a level in laryngeal cancer was analyzed by receiver characteristic curve(ROC).Results The expression level of let-7a in laryngeal cancer patients was compared in different degrees of differentiation,TNM stage and lymph node metastasis(P<0.05),and in gender,age,tumor type,smoking history and drinking history(P>0.05).ROC curve analysis showed that the area under the curve(AUC)was 0.851,the 95%confidence interval was 0.775~0.933,the sensitivity was 73.9%,the specificity was 78.2%,and the optimal critical value of let-7a relative expression was 0.77.Conclusion let-7a level presents abnormally low expression in patients with laryngeal cancer,and its level change is related to the differentiation degree of patients,TNM stage and lymph node metastasis,and can be used as a sensitive indicator to predict the risk of laryngeal cancer.
