甲氧西林耐药及敏感金黄色葡萄球菌快速鉴定中梅里埃VTIEK MS质谱仪的应用价值探究

摘　　要：目的探究甲氧西林耐药金黄色葡萄球菌及敏感金黄色葡萄球菌快速鉴定中梅里埃VTIEK MS质谱仪的应用价值。方法随机选取2022年1月—2023年1月南京医科大学康达学院附属涟水县人民医院收治的合并感染的骨科患者80例作为研究对象,均接受迪尔96L及药敏板、梅里埃VITEK MS质谱鉴定。观察梅里埃VITEK MS质谱仪的应用价值。结果随机选择患者分离出来的金黄色葡萄球菌80株,其中耐甲氧西林金黄色葡萄球菌(MRSA)42株(52.50%),甲氧西林敏感金黄色葡萄球菌(MSSA)38株(47.50%)。分别从分离出来的金黄色葡萄球菌中抽取MRSA 22株、MSSA 20株,共找出两个MRSA主要特征峰,即2305.6 Da和3007.3 Da。另外随机利用其他的MRSA 20株、MSSA 20株对该模型展开验证。比对MIC法,结果提示特征峰1灵敏度为78.95%,特异度为76.19%;特征峰2灵敏度为82.35%,特异度为73.91%;联合两点灵敏度为73.91%,特异度为82.35%。结论MRSA及MSSA快速鉴定中,梅里埃VTIEK MS质谱仪具有更好的特异性及灵敏度。Objective To explore the application value of Mérieux VTIEK MS mass spectrometer in the rapid identification of methicillin-resistant staphylococcus aureus and sensitive Staphylococcus aureus.Methods Randomly select-ed 80 orthopedic patients with concurrent infections admitted to Lianshui County People's Hospital affiliated with Kangda College of Nanjing Medical University from January 2022 to January 2023 as the study subjects.All were identified by Dil 96L and drug sensitive plate,Mérieux VITEK MS mass spectrometry.Observed the application value of the Mérieux VITEK MS mass spectrometer.Results 80 strains of staphylococcus aureus were isolated from randomly selected patients,including 42 strains(52.50%)of methicillin-resistant staphylococcus aureus(MRSA).There were 38 strains(47.50%)of methicillin-sensitive staphylococcus aureus(MSSA).22 strains of MRSA and 20 strains of MSSA were extracted from the isolated Staphylococcus aureus,and two main characteristic peaks of MRSA were found,namely 2305.6 Da and 3007.3 Da.In addition,other 20 strains of MRSA and 20 strains of MSSA were randomly used to verify the model.Compared with the MIC method,the results indicated that the sensitivity of characteristic peak 1 was 78.95%,and the specificity was 76.19%.The sensitivity of characteristic peak 2 was 82.35%,and the specificity was 73.91%.Combining the two points,the sensitivity was 73.91%,and the specificity was 82.35%.Conclusion In the rapid identification of MRSA and MSSA,the Mérieux VTIEK MS mass spectrometer has better specificity and sensitivity.
