阿尔兹海默病患者血清LP(a)、sCD22、Hcy水平与认知功能的相关性

摘　　要：目的分析阿尔兹海默病(AD)患者血清脂蛋白a(LP(a))、可溶性CD22(sCD22)、同型半胱氨酸(Hcy)水平与认知功能的相关性。方法回顾性选取西安市第五医院及西安交通大学第一附属医院2020年3月至2022年3月收治的98例AD患者为研究对象,根据简易精神状态检查量表(MMSE)评估结果将患者分为40例轻度组、32例中度组、26例重度组。比较三组患者血清LP(a)、sCD22、Hcy水平,同时采用Pearson相关性分析AD患者血清LP(a)、sCD22、Hcy水平与认知功能的相关性。结果随着AD患者病情程度的加重,其MMSE总评分和各项MMSE评分逐渐降低,差异均有统计学意义(P<0.05);而血清LP(a)、Hcy水平随着病情程度的加重而逐渐升高,sCD22水平随着病情程度的加重而逐渐降低,差异均有统计学意义(P<0.05);经Pearson相关性分析结果显示,AD患者血清LP(a)水平与MMSE评分呈正相关(r=0.327,P<0.05),sCD22水平与MMSE评分呈负相关(r=-0.428,P<0.05),Hcy水平与MMSE评分呈正相关(r=0.459,P<0.05)。结论AD患者随着病情加重,血清LP(a)、Hcy水平显著升高,sCD22显著降低,且血清LP(a)、sCD22、Hcy水平与AD患者认知功能相关,建议临床对其进行密切监测。Objective To analyze the correlation between serum lipoprotein a[LP(a)],soluble CD22(sCD22),homocysteine(Hcy)levels and cognitive function in patients with Alzheimer's disease(AD).Methods A total of 98 pa-tients with AD admitted to Xi'an Fifth Hospital and the First Affiliated Hospital of Xi'an Jiaotong University from March 2020 to March 2022 were selected retrospectively.According to the Mini-Mental State Examination(MMSE)score,the patients were divided into mild group(40 cases),moderate group(32 cases),and severe group(26 cases).Serum LP(a),sCD22,and Hcy levels in the three groups were comparatively analyzed.Pearson correlation analysis was performed to an-alyze the correlation between serum LP(a),sCD22,Hcy levels and cognitive function in patients with AD.Results The total score and the item scores of MMSE decreased with the aggravation of AD,with statistically significant differences(P<0.05).Serum LP(a)and Hcy levels increased and sCD22 Level decreased with the aggravation of AD,with statistical-ly significant differences(P<0.05).Pearson correlation analysis results showed that serum LP(a)level was positively corre-lated with MMSE score in patients with AD(r=0.327,P<0.05),sCD22 level was negatively correlated with MMSE score(r=-0.428,P<0.05),and Hcy level was positively correlated with MMSE score(r=0.459,P<0.05).Conclusion With the aggravation of AD,serum LP(a)and Hcy levels increase significantly,while sCD22 level decreases significantly.The three indexes are closely related to cognitive function in patients with AD.It is recommended that they be closely moni-tored in clinical practice.
