摘 要:目的探索术中导航3D导板辅助MPFL重建治疗复发性髌骨脱位的膝关节运动学情况。方法2018年1月—2020年12月符合入选标准的60例患者,随机分为3D导板组(3D组)和常规组,2组的性别占比,年龄无显著差异,对2组患者的术前、术后3、6、9、12个月患膝关节活动的六个自由度进行测量、对比分析,同时与30例健康成人膝关节运动学数据进行对比研究。结果2组患者在术后3个月屈伸角度范围相比无差异(P>0.05),但均小于健康成人(P<0.05),3D组患者在术后6、9个月的屈伸角度范围显著优于常规组(P<0.05),3D组与健康成人相比无差异(P>0.05),常规组在术后6、9个月屈伸角范围小于健康成人(P<0.05)。结论膝关节运动学分析仪能更加客观、动态评估膝关节的运动学及功能;应用3D打印导板辅助行MPFL重建可获得更满意的膝关节运动学功能。Objective To explore knee joint kinematics of intraoperative navigation guide plate-assisted MPFL reconstruction in the treatment of recurrence patella dislocation.Methods Sixty patients met the inclusion criteria from Jan.2018 to Dec.2020,who randomly divided into 3D guide plate group and conventional group.There was no significant difference in ration of gender and age in two groups.Measured and analyzed six freedom degrees of knee joint motion of patients in two groups before surgery,3,6,9,12 months and after surgery.At the same time,compared with knee joint motion data of 30 healthy adults.Results There was no difference in the range of flexion and extension angle between the two groups 3 months after the operation(P>0.05),but both of them were smaller than that of healthy adults(P<0.05).The range of flexion and extension angle in the 3D guide group was significantly better than that in the conventional group 6 and 9 months after operation(P<0.05),and there was no difference between the 3D guide group and healthy adults(P>0.05).Conclusions Knee kinematics analyzer can evaluate the kinematics and function of knee joint more objectively and dynamically.More satisfactory knee joint kinematics function can be obtained by using 3D printing guide plate to assist MPFL reconstruction.
