摘 要:目的探讨乳腺癌及其分子亚型的雄激素受体(androgen receptor,AR)表达与^(18)F-FDG摄取的相关性。方法选取143例女性患者经穿刺活检或手术病理确诊为乳腺癌,且均在治疗前接受了^(18)F-FDG PET/CT检查,对乳腺癌病灶勾画感兴趣区,获得相应的最大标准摄取值(maximum standardized uptake value,SUVmax)以反映乳腺癌的^(18)F-FDG摄取水平。根据ER、PR、HER-2和Ki-67的免疫组化和荧光原位杂交(fluorescence in situ hybridization,FISH)结果进一步将乳腺癌分为Luminal A型、Luminal B(HER-2阴性)型、Luminal B(HER-2阳性)型、HER-2过表达型、三阴型。AR的表达也根据免疫组化染色评价,阳性细胞数<10%为阴性(-),10%~25%为(1+),26%~50%为(2+),>50%为(3+)。结果乳腺癌AR总体阳性表达率77.62%,Luminal A型、Luminal B(HER-2阴性)型、Luminal B(HER-2阳性)型、HER-2过表达型、三阴型AR阳性表达率分别为96.15%、71.88%、93.02%、93.75%、30.77%。三阴型AR阳性率显著低于其他各型(P<0.05),Luminal B(HER-2阴性)型AR阳性率低于Luminal A型和Luminal B(HER-2阳性)型,差异有统计学意义(P<0.05)。分析乳腺癌总体的^(18)F-FDG摄取,AR阴性乳腺癌的SUVmax高于AR阳性乳腺癌(8.36 vs 5.68,P<0.05);分析各分子亚型的^(18)F-FDG摄取,三阴型中AR阴性乳腺癌的SUVmax显著高于AR阳性乳腺癌(11.28 vs 4.42,P<0.05)。乳腺癌及其分子亚型AR阳性1+、2+组和AR阳性3+组的SUVmax差异无统计学意义(P均>0.05)。结论三阴型乳腺癌的AR阳性表达率最低,其^(18)F-FDG摄取水平可以反映AR的表达状态。Objective To explore the correlation between^(18)F-FDG uptake and androgen receptor in breast cancer and its molecular subtypes.Methods A total of 143 female patients(age ranged from 27 to 90 years,with a median age of 52 years)were pathologically diagnosed with breast cancer by biopsy or surgery.They all undertook^(18)F-FDG PET/CT before treatment.By drawing regions of interest on breast cancer lesions,the maximum standardized uptake value(SUVmax)was obtained to reflect the^(18)F-FDG uptake level.According to the status of ER,PR,HER-2 and Ki-67,breast cancer was further classified as Luminal A subtype,Luminal B(HER-2 negative)subtype,Luminal B(HER-2 positive)subtype,HER-2 overexpressing subtype and triple negative subtype.The AR status was determined by immunohistochemical staining,which was defined as negative if positive cells were less than 10%,as(1+)if positive cells accounted for 10%-25%,as(2+)if positive cells accounted for 26%-50%and as(3+)if positive cells were more than 50%.Results The total positive expression rate of AR for breast cancer was 77.62%.The rate was 96.15%,71.88%,93.02%,93.75%and 30.77%,respectively for Luminal A subtype,Luminal B(HER-2 negative)subtype,Luminal B(HER-2 positive)subtype,HER-2 overexpressing subtype and triple negative subtype.The positive expression rate of AR for triple negative subtype was significantly lower than that for other subtypes(all P less than 0.05).The rate for Luminal B(HER-2 negative)subtype was lower than that for Luminal A subtype and Luminal B(HER-2 positive)subtype(P<0.05).As for the total^(18)F-FDG uptake of breast cancer,the SUVmax of breast cancer with negative AR was higher than that with positive AR(8.36 vs 5.68,P<0.05).As for the^(18)F-FDG uptake of various molecular subtypes,the SUVmax of the triple negative subtype with negative AR was significantly higher than that with positive AR(11.28 vs 4.42,P<0.05).There was no statistical significance concerning SUVmax between the group with 1+,2+AR and the group with 3+AR,either for breast cancer or for va
