摘　　要：目的比较LI-RADS 2018版和2017版的MRI诊断效果。方法选取115例肝细胞肝癌患者,包括121个肝脏局灶病变。由3位MRI诊断医师独自分析、分别根据LI-RADS 2018和2017进行LR分类,再做统计分析。结果当LR-5作为预测指标时,LI-RADS 2018与2017版相比,敏感性(63.1%vs.53.8%),阴性预测值(69.2%vs.64.3%)和准确性(78.5%vs.73.4%),约登指数(0.595vs.0.502);当LR-4/5为预测指标时,MRI诊断HCC的诊断价值差异无统计学意义(P>0.05)。2017版中7个LR-4病灶在2018版中分类为LR-5。结论LI-RADS 2018版诊断性能优于2017版,具有更高的敏感性,阴性预测值和准确性。当LR-5为肝细胞肝癌的预测因子时,2018版的修订主要影响LR分类。Objective To ccompare MR diagnostic results of the versions of LI-RADS 2018 and 2017.Methods 115 patients,including 121 focal liver lesions,admitted to our hospital,were analyzed retrospectively.Three MRI diagnosticians independently analyzed and classified LR according to LI-RADS 2018 and 2017 respectively,and then made statistical analysis.Results Compared with the 2017 version,LI-RADS 2018 had sensitivity(63.1%vs.53.8%),NPV(69.2%vs.64.3%)and accuracy(78.5%vs.73.4%%),and Yoden index(0.595).When LR-4/5 was the predictor,the diagnostic value of HCC on MR was not statistically significant.7 LR-4 in 2017 version were classified as LR-5 in 2018 version.Conclusion The diagnostic performance of LI-RADS 2018 is better than that of 2017,and which it has higher sensitivity,NPV and accuracy.When LR-5 is a predictor of HCC,the revision in 2018 mainly affects LR classification.
