药物诱发急性闭角型青光眼发作的临床分析

摘　　要：目的探讨诱发急性闭角型青光眼发作的药物种类以及诱发机制。方法选取2019年2月至2021年2月本院眼科收治、院内会诊及院外会诊的药物使用后诱发的11例(17眼)急性闭角型青光眼患者,分析患者原发疾病、药物使用情况、临床特点、诊疗经过及预后情况。结果11例患者中,双眼发病6例,单眼5例。诱发药物:胆碱受体阻断药物4例,抗帕金森病药物1例,硝酸酯类药物1例,三环类抗抑郁药物1例,呼吸系统药物2例,磺胺类药物1例,头孢类抗菌药物1例。结论多种药物有诱发急性闭角型青光眼发作的可能,临床用药需谨慎,针对急性闭角型青光眼发作患者,及时发现,早期积极治疗,是挽救视功能的关键。Objective To explore the drug types and mechanism of drug induced acute angle-closure glaucoma.Methods 11 patients(17 eyes)with acute angle-closure glaucoma induced by drug use were selected from the Department of Ophthalmology of our hospital,in-hospital consultation and out-hospital consultation from February 2019 to February 2021.The primary disease,drug use,clinical characteristics,treatment course and prognosis of the patients were analyzed.Results Among 11 cases patients,there were 6 cases of bilateral glaucoma and 5 cases of monocular glaucoma.Inducing drug:there were 4 cases of anticholinergic medications,1 case of antiparkinsonian medications,1 case of nitrates,1 case of tricyclic antidepressants,2 cases of respiratory system drugs,1 case of sulphonamide-derived medications,1 case of cephalosporins antibiotics.Conclusion Many drugs may induce the acute angle closure glaucoma.These drugs should be cautious,for patients with acute angle closure glaucoma,timely detection and early active treatment are the key points to save the visual function.
