荧光定量聚合酶链式反应法与免疫组织化学法检测胃黏膜活检标本中幽门螺旋杆菌感染的价值分析

摘　　要：目的分析荧光定量聚合酶链式反应(PCR)法与免疫组织化学法检测胃黏膜活检标本中幽门螺旋杆菌(HP)感染的价值。方法选取2019年2月至2021年3月本院收治的184例慢性胃炎患者的病理活检标本作为研究对象,采用荧光定量PCR法与免疫组织化学法检测患者HP感染情况。比较两组检测方式诊断效能、HP感染强度。结果荧光定量PCR法阳性率为30.43%,阴性率为69.57%;免疫组织化学法阳性率为25.00%,阴性率为75.00%,两种检测方式诊断价值比较差异无统计学意义。免疫组织化学法和荧光定量PCR法对HP感染强度的Kappa值为0.591,检测一致性一般;荧光定量PCR法比免疫组织化学法分布更均匀,差异有统计学意义(P<0.05)。结论荧光定量PCR法和免疫组织化学法均可检测HP感染,荧光定量PCR法对轻中度HP感染的检出效果更好。Objective To analyze the value of real-time polymerase chain reaction(PCR)and immunohistochemistry in the detection of Helicobacter pylori(HP)infection in gastric mucosal biopsy specimens.Methods A total of 184 pathological biopsy specimens of patients with chronic gastritis admitted to our hospital from February 2019 to March 2021 were selected as research subjects,and they were detected by fluorescence PCR and immunohistochemistry.The diagnostic efficacy and HP infection intensity of the two groups were compared.Results The positive rate of PCR was 30.43%,and the negative rate was 69.57%.The positive rate of immunohistochemistry was 25.00%and the negative rate was 75.00%,and there was no significant difference in the diagnostic value between the two detection methods.The Kappa value of immunohistochemistry and real-time PCR on the intensity of HP infection was 0.591,and the detection consistency was average,and the distribution of real-time PCR was more uniform than that of immunohistochemistry,and the difference was statistically significant(P<0.05).Conclusion Both real-time PCR and immunohistochemistry can detect HP infection,and the detection of mild to moderate HP infection by real-time PCR is more effective.
